The drill operator was assisting on drill maintenance, EE was guiding the drill hammer which was suspended by the mechanic truck boom when EE tripped over a rock, that had not been removed from the work area. The operator stumbled while maintaining EEs grip on the drill hammer, between EEs grip on the hammer and EEs shifting body weight resulted in a fracture to the left scaphoid.

While operating the telescopic forklift, the miner opened the left-hand door, high wind pulled the door from the miner's grip, broke safety strap, the door hit the frame and shattered the glass creating flying debris. Debris was removed from beneath the eyelids, an ointment applied, the mine returned to work.

While descending the steps from the top level of the plant, the Miner slipped with wet boots causing EE to hit head below EE's hardhat strap. When EE reported the incident to the Foreman, EE became ill and vomited. The supervisor called emergency services.

A contractor entered the site during non-working hours to retrieve a personal car spring from EE's personal storage trailer. A guest that was accompanying the contractor heard a thud as back was turned towards the trailer. The guest turned and found the contractor lying on the ground with head injuries.

While cleaning up after a welding repair in sizing screen, the Miner transferred their weight onto their left arm and rotated to swing their legs over side of sizing screen to exit. This caused the Miner to strain their left shoulder.

The Miner was found unresponsive in front of their parked Caterpillar 777D Haul truck. They reported that they don't remember anything after descending the upper ladder of the truck.

While shoveling under a conveyor, the Miner's right hand struck the expanded metal tail guard on the forward stroke causing a cut between the thumb and pointer finger. The cut required 4 stitches to close.

While installing a pin in the bucket cylinder of the PC2000, the Miner had their foot positioned under a pallet board. The pin came loose from its rigging and struck the pallet which in turn struck the Miner's left foot behind the steel toe shoe.

While operating the Caterpillar 785B haul truck, the Miner injured their back when the seat bottomed out when a dip in the road was encountered.

While drilling production blast holes to the coal basin on the Marion 8700 Dragline Bench, an unplanned release of carbon monoxide and hydrogen sulfide inundated the pit area. The Miner was overcome by the gasses and became nauseated. Employee was taken to the hospital where they tested positive for elevated levels of carbon monoxide in their blood.

While helping to repair/splice the fuel line on the International 4900 Fuel Truck, the repair blew out from the hose clamps under pressure. The Miner was struck in the groin with the hose causing tissue damage.

After filling the radiator with fluid on the Epiroc D65 Drill and returning the tools to the grease truck, the Miner returned to replace the radiator cap. Mud on Miner's boots caused the Miner's feet to slip and fall from the ladder landing on right leg, spraining knee.

While cleaning raw coal material from on top of the locked out Raw Coal Stacking Conveyor, the Miner was standing on the conveyor ahead of the conveyor feed chute. The belt and material moved backwards, sweeping the Miner into the chute and covered Miner with material.
