Showing all 47Employee was working in an awkward position trying to replace a bad hydraulic hose for multiple hours. EE was laying on EE's stomach and reaching into the access door. After work was completed, employee reported a back strain.
The longwall operators were cutting high in the tailgate causing the shear to cut out roof support (bolts). This roof fall occurred in the tailgate (bleeder) no employees were trapped, injured, and air was not disrupted by the event.
Employee was operating a forklift steering wheel with EE's left hand when EE felt a pop in EE's shoulder.
A roof fall occurred in the longwall tailgate that went above anchor point. Ventilation was not interrupted, no miners were trapped, and no injuries occurred. The area where the roof fall occurred is a bleeder and is not a working area but the roof fall did extend passed the anchor point.
The employee was operating a warm water return valve and felt pain in their lower back.
Employee opened an angle cock valve on the back of a rail car to bleed pressure. while opening the valve the hose whipped up and struck their elbow.
Working on the mono drop-out line had the guzzler hose down into the ditch, propped up on EE's knee, then EE twisted EE's knee and felt a pop.
The incoming power from Rocky Mountain Power had a double tie line that tripped power. This event was not produced by any equipment or process failures on WeSoda side. RMP sent linemen out to address and fix the issue.
EE reached out with EE's left arm to lift up a rolled up hose hanging on a hook at 6' 4" and felt a pop in EE's elbow. When EE sat the hose down EE's elbow popped again.
A piece of longwall equipment was loaded on the cage that exceeded the weight compacity by 500lbs and while it was being hoisted out of the mine the motor faulted out causing the breaks to set while in motion and the ropes slipped.

Employee was shoveling along side the crusher when a rock 28" x 30" x 4 1/2" fell and landed on their ankle. Causing a sprain/strain to their ankle.

while turning power back on during a planned outage the generator for hoist shut down causing the hoist computers to shut down. This required the hoist computers to be rebooted causing the sync switch to fault out. The hoist was down for 37 minutes.
There was a roof fall in the tailgate of the longwall measuring 15ft to 20ft long and 7ft to 8ft high. The fall did not restrict ventilation, cause injury, or impede escapeways.
The miner was cleaning up the walkway when EE picked up a large rock and felt a pop in EE's back.

there was an above anchor point fall above the overcast near 2 northwest pump station. the fall damaged the overcast causing it to outgas.
Employees were drilling the hole when the pipe started to slip. Employee grabbed another wrench to put it on the pipe, while employee was setting the wrench they had a hold of the other pipe. The wrench slipped and the one they were holding accidentally grabbed and smashed their hand between the wrench and the mass of the drill.
Employee was carrying pin rollers when they tripped and fell on their shoulder. Employee tripped over grating outside the 4shaft airlock.
Madsen (contractor) Front end loader operator was cleaning material out of mono loadout silo 2. The material above the entryway came down on the front of the front-end loader and pushed the steering column and dash down into the operators' legs trapping EE in the loader.
While welding grating the bin over flowed dropping a small rock into the hood contacting right eye.

Lowering a piece of grating and felt a pop in right elbow. Seen at on-site medical facility and released. Still felt pain a few days later and went to personal physician on 5/27/25 where it was found EE had a torn right bicep
Slip and fall when pulling wrench off bolt. Fell and landed on a rock in the employee's lower back.
Employee was putting drill steel in the chuck while hand was in a pinch point and closed the centralizer on their hand.
Employee was walking on an uneven surface and felt pain in knee, sought medical attention and was released back to work. On 05/10/2025 employee was still experiencing pain sought medical attention and was put off of work until an MRI can be performed.
The 5 hoist mantrip had an unknown electrical issue which deemed it inoperable all miners were evacuated.
Employee wash unplugging a caustic line and got liquid on their pants causing a caustic burn. Employee went for a follow up appointment on 04/26/2025 and was put off work for one day.
The operator was adjusting the lime drag when they pinched a finger, resulting in a laceration that required 11 sutures.
