The injured employee stated that while walking behind a scoop EE stepped on a rock concealed by rock dust, causing EE to twist EE's left ankle and sustain a fracture.

High Carbon Processing, LLC · Accident type, without injuries

An unplanned fire not extinguished within 30 minutes of discovery occurred on a Caterpillar 325c excavator on the course refuse disposal area of the preparation plant. The machine was owned and operated by Doss Enterprises, LLC. No injuries resulted from the incident.

Injured employee was attempting to install a roof bolt when the bolt bent, separated from the drill chuck, and struck EE's right forearm resulting in a fracture and laceration.

While watering belt to reduce friction against skirt rubber during a changeout, employee tripped over the water hose as EE was moving down the cat walk. EE fell and broke EE's wrist.

Employee stated that while cleaning on the slope belt they tripped and fell, striking components of the slope belt, resulting in facial fractures.

Injured employee was walking in the #2 entry of the HG 5 section when EE stepped in a hole and twisted EE's left knee. No scheduled shifts have been missed as a result of the incident, employee started physical therapy on 4/27/26.

Employee was observing co-workers repair continuous miner conveyor chain when a piece of metal struck their nose. Medical treatment was required on 2/23/26. No time was missed as a result of the incident.

Employee stated that while pulling belt structure they twisted and felt pain in their right knee. Employee received medical treatment on 3/6/2026 for the injury. Employee has not missed any scheduled shifts as a result of the incident.

Injured employee was attempting to push a rock off the cable tray of the longwall panline when EE caught EE's left index finger between the rock and shield canopy resulting in a fracture to the tip of EE's finger and a laceration requiring sutures to close. No time was missed as a result of this incident.

Employee was working in soft ground conditions when EE's right foot became stuck. EE lost balance, fell, and fractured EE's ankle. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

A dozer entered void on the #2 Met Stockpile at the #3 Feeder resulting in an entrapment. No injuries resulted.

Allegheny Metallurgical, LLC · Fall from ladders

Employee set up a 5' step ladder to assist with hanging a bag. When they stepped up onto the first rung the left leg shifted causing them to lose balance and fall. They used their left arm to brace for the fall. Their hand struck the ground spraining their thumb. Injury was not the result of a training or compliance issue.

The emergency hoist was damaged during routine testing resulting in the hoist being removed from service for a period of greater than 30 minutes.
