While walking across the cat walk of a 777 the miner slipped and fell to the ground approximately twelve feet landing on his right side. The result was 4 broken ribs and a bruised lung.

While driving in a bogey pin into a D-10 bogey on a side frame, using a sledge hammer the victim partially struck the track frame which caused him to strike himself in the lower right leg. This was a struck by injury. He was treated and released.

While attempting to turn the bumper of the lube truck, on the forks of the lift, using a support leg, the bumper shifted and rolled off the forklift onto the victim's leg. The victim was taken by fellow employee, to the hospital e.r. for treatment. Injury was minimal. EE was released to return to work the next day.

While hauling & piling topsoil; reclaiming, with TS 24 Pan, rear slipped and began to slowly roll over. Pan rolled completly over once with operator buckled in. Pan then set upright long enough for operator to unbuckle and dismount. Pan continued to roll down slope. Operator landed on right shoulder & right side ribs.

While placing a pull cable on a D11 Dozer, miner said he felt a pain in the upper inside of right leg (groin area). Miner did not seek treatment until 3/18/10 in ER, for soreness and discoloration of affected area.

Miner was pulling 4" hose up steep bank and his right knee popped out of place then popped back in. Went to take another step and right knee popped out again. Miner waited a little bit and rested the knee, went back to work. Miner states his right knee has been hurting ever since, and on 7/11/09 he sought medical treatment in hospital ER.

While trying to keep the fuel pump motor running, employee had to climb onto Fuel Truck to start. Employee slipped and fell off back of Fuel Truck onto his right side. There was a significant sleet accumulation and freezing rain during this shift. Employee was seriously injured. Fractured Collarbone, sternum, 1 fractured rib, 2 fractured back vertebra.

After fueling Cat 345 Hoe (excavator) miner trying to dismount machine, his foot slipped off due to ice & snow on machine, and other foot slipped on frozen ground, landing miner on his back. The miner states it hurt his lower neck and lower back. Miner continued working until end of shift on 1/2/08.

ee was getting down off loader, his foot slipped on top platform which he had only his left hand on handrail.Couldn't get right hand on guard rail in time. The fall while holding onto the handrail with one hand caused him to swing into platform hitting rib cage against platform. He then lost grip and fell to the ground.

AS THE CARRIAGE WAS MOVING FORWARD TO GET ANOTHER FLIGHT, THE EE POSITIONED HIMSELF BETWEEN A STATIONARY LIFT JACK & THE MOVING CARRIAGE WHICH PINCHED HIM & FRACTURED HIS PELVIS. THE OPERATOR HEARD HIM YELL & STOPPED THE CARRIAGE.

HE WAS DISMOUNTING FROM THE CAT D10L DOZER WHEN HE LOST HIS FOOTING WHILE STEPPING ON THE PUSH ARM. WHEN HE STARTED TO FALL, HE GRABBED AND CAUGHT HIMSELF BY HOLDING ONTO THE HAND RAILING. THE WEIGHT OF HIS BODY BEING SUPPORTED BY HIS LEFT ARM CAUSED HIS LEFT BICEP TO TEAR REQURING SURGERY.

Was walking across the shop parking area headed to the office at 6:00 am when it was dark. another ee was backing pick-up truck out of shop bay toward office. the driver of the truck said he was dark and he did not see EE walking toward the office tuntil he hit him, knocking him to the ground. hit in left shoulder area.

He was repairing the exhaust on the michigan loader. he was standing on the right rear tire and bent over to see if the other was handing him the new exhause pipe, slipped and fell to the ground.

His dozer was stuck in the mud so he got another operator on another dozer to help pull him out. He was pulling a cable by hand over rock, and mud trying to get it hooked onto his dozer. While bending and pulling the cable he felt a sharp pain in his lower back. He didn't report it to his doctor for three days after the injury.

EE WAS RIDING ON THE SALEM AUGER WHILE A LOADER PULLED & A DOZER PUSHED THE MACHINE UP OUT OF THE PIT. THE PIN HOLDING THE HITCH TO THE LOADER BROKE CAUSING THE AUGER TO TILT FORWARD DROPPING ABOUT FOUR FEET. THE FALLINGOF THE MACHINE JARRED THE EE'S NECK, BACK AND RIGHT SHOULDER WHEN IT LANDED.

EMPLOYEE WAS ASSISTING MECHANIC PUT A ROLLER ON D-10L DOZER. HE WELDED A BOGIE PIN TO THE FRAME TO GET ROLLER UNDER MACHINE WHEN THE WELD BROKE WHILE HE WAS PUTTING THE ROLLER IN PLACE. THE RO LLER BOGIE FELL DOWN, PINCHING HIS RIGHT ARM BETWEEN THE BOGIE AND A ROLLER ON THE GROUND.

IT WAS COLD AND THE GROUND WAS SNOW COVERED WHEN EE WAS WALKING FROM HIS SERVICE TRUCK TO A MACHINE WHEN HE SLIPPED FROM STEPPING ON A MACHINE PART THAT WAS ON THE GROUND UNDER THE SNOW. THE S LIP CAUSED HIM TO SPRAIN HIS LEFT ANKLE.

WHILE DISMOUNTING FROM HIS MACHINE HIS HAND SLIPPED OFF THE HANDHOLD AND HE GRABBED A SHARP PIECE OF THE CATWALK SUPPORT RAIL TO PREVENT HIMSELF FROM FALLING. THE SHARP METAL PIERCED HIS PALM CAUSING HIM TO GET SIX (6) STITCHES IN HIS LEFT HAND.
