Struck by falling object

While attempting to turn the bumper of the lube truck, on the forks of the lift, using a support leg, the bumper shifted and rolled off the forklift onto the victim's leg. The victim was taken by fellow employee, to the hospital e.r. for treatment. Injury was minimal. EE was released to return to work the next day.

Miner tripped over tounge of brush hog, causing him to fall forward catching himself with his arms, he wrenched/landed wrong on his right shoulder.

Fall to the walkway or working surface

Injured worker was walking from Main office to Training Trailer, slipped on snow covered ice, and fell hard to concrete with bodyweight landing on right leg and foot.

Caught in, under or between running or meshing objects

Employee was attempting to install new belt on portable air compressor. Left ring finger was pulled between belt and roller, smashed flat, with nail avulsion, fracture to fingertip, and possible nerve & tendon damage.

Fall from machine

Loading auger on to truck. Loader man was pushing with loader and when he pulled back from auger the part that the man was standing on fell 1 to 2 feet and he flipped over.

Struck by flying object

Ice & snow covered highwall, thawing. EE was unpinning auger flite approx 4' from highwall. Big rock fell from highwall, hit auger, rock guard, pipe, split, 1/2 rock hit EE's left leg @ knee area.

IT WAS RAINING AND HE WAS GETTING INTO A 1989 MACK TRUCK/TRACTOR WHEN HE LOST HIS FOOTING AND FELL TO THE GROUND AND LANDED ON HIS RIGHT WRIST.

Was walking across the shop parking area headed to the office at 6:00 am when it was dark. another ee was backing pick-up truck out of shop bay toward office. the driver of the truck said he was dark and he did not see EE walking toward the office tuntil he hit him, knocking him to the ground. hit in left shoulder area.

Struck against a moving object

EE WAS RIDING ON THE SALEM AUGER WHILE A LOADER PULLED & A DOZER PUSHED THE MACHINE UP OUT OF THE PIT. THE PIN HOLDING THE HITCH TO THE LOADER BROKE CAUSING THE AUGER TO TILT FORWARD DROPPING ABOUT FOUR FEET. THE FALLINGOF THE MACHINE JARRED THE EE'S NECK, BACK AND RIGHT SHOULDER WHEN IT LANDED.

EE WAS CLEANING RUST OFF AIR BAG BRACKET WITH AN AIR GRINDER WHEN THE WHEEL CAME APART AND STRUCK EE IN THE LEFT ARM. THE LACERATION REQUIRED SUTURES.

Struck by falling object

EE WAS REMOVING A TIRE FROM AN OLD SCRAP MECHANIC TRUCK WHEN THE DOOR TO THE UTILITY BED FELL DOWN AND LANDED ON HIS HEAD. THE EDGE OF THE DOOR CAUSED A LACERATION REQUIRING 6 SUTURES.

Employee was assisting another load auger flights onto a flatbed truck using a loader. Employee was the ground man directing the flights onto the truck. The loader operator lost sight of him and hit his head with the bucket.

EE WAS USING AN AIR HOSE TO BLOW DEBRIS OFF THE BUMPER OF HIS TRUCK WHEN A PIECE OF METAL/DIRT FLEW INTO HIS LEFT EYE. WENT TO DR. TO GET FOREIGN OBJECT REMOVED.

EE WAS STEPPING DOWN OFF A 50 TON ROCK TRUCK ANDHIS FOOT ROLLED WHEN HE LANDED IN A LOW AREA OF THE GROUND CAUSING HIS LEFT KNEE TO TWIST.

EMPLOYEE WAS CLEANING OUT THE BED OF THE TRUCK AND WE ATTEMPTED TO GET OUT OF THE TRUCK, HE SLIPPED AND HIS LEFT HAND WENT ONTO A BOLT WHICH WASHOLDING A VICE ONTO THE TRUCK BUMPER CAUSING A CONTUSION WHICH REQUIRED SUTURES.

IT WAS COLD AND THE GROUND WAS SNOW COVERED WHEN EE WAS WALKING FROM HIS SERVICE TRUCK TO A MACHINE WHEN HE SLIPPED FROM STEPPING ON A MACHINE PART THAT WAS ON THE GROUND UNDER THE SNOW. THE S LIP CAUSED HIM TO SPRAIN HIS LEFT ANKLE.

EE WAS USING A GRINDER WITH A WIRE WHEEL TO CHIPOFF PAINT ON A PORT SO HE COULD WELD IT. ONE OF THE WIRES FROM THE WHEEL BROKE LOOSE & WENT INTOHIS RIGHT FOREARM. HE DIDN'T NOTICE IT FOR A FEW DAYS & THEN BEGAN TO HURT. HE WENT TO THE DOCTORTO HAVE IT REMOVED.
