A rock fall striking EE on right leg while EE was operating continuous miner.
EE was helping load structure and dropped one end of the structure and it landed on right hand striking right thumb.
Employee was pulling miner cable when a rock fell on left ankle. Took to Pikeville Medical Center for evaluation of injured ankle, x-ray showed no major injury, nothing broken. Employee received 3 days off - paid by employer - Saturday, Monday, & Tuesday.
Employee was lifting on belt putting structure in.
EE was not feeling well before going underground. He rode in on his 4-wheeler, went to the feeder told the shuttle car operator he was not feeling well so he was going to leave to go home, but he would stop and check his drive on the way out. Said he didn't do anything to hurt himself but hurt worse as he rode.
EE was operating a ram car. He was turning a corner with his arm outside the operators deck. He turned the corner and the trailer caught his right arm between a pinch point of the ram car.
Employee said he hit the rib with the 21SC and he hit his head on the canopy.
Lid covering pick breaker chain on feeder was raised up and braced so work could be done, when brace slipped letting lid fall suddenly hitting EE on finger of left hand.
EE was in process of tramming double head roof bolter into the #4 heading. After arrival in heading a rock measuring 1x1x3/4 fell from mine roof, striking ee on left hand, resulting in hairline fracture of left thumb.
EE said he was walking away from his 21SC. He then stopped on the corner, put his hand on the rib and rock rolled off the rib striking him on his right side. Witnesses from 3rd shift state that they pulled this rib prior to the beginning of the shift.
Employee lifted a case of bottled water - lower back pain.
Was loading clean up car and rock fell on miner.
Rib rolled off striking employee across right shoulder and back.
Substation on surface caught fire could not be extinguished. Internal comments of the substation were destroyed by fire.
EE said before shift started he was hurting in groin area and stomach while operating the 21sc shuttle car when he hit a hole in the roadway that made him hurt worse.
THERE WAS A ROCK IN THE FEEDER. EE WAS GOING TO GET THE ROCK OUT. SUPERVISOR WAS GOING TO MOVE SCOOP OUT OF THE WAY AND THERE WAS A MISCOMMUNICATION. EE GOT IN THE FEEDER WHILE SUPERVISOR WAS MOVING SCOOP AND THE SCOOP GOT HIS ANKLE.
EE had a 930 cat loader brake head in his vehicle when he went to get the brake head out of the vehicle he pulled a muscle in his lower back.
INJURED EMPLOYEE WAS TRYING TO GET A PIECE OF ROCK OFF MINER WITH A SLATE BAR WHILE TRYING TO GET THE ROCK OFF MINER HE FELT OR HEARD SOMETHING TEAR IN HIS LOWER BACK.
Out by area intersection of spad #1148, #3 entry. 50'long x 20'wide x 5-6'thick. Cribbed Off.
FACE AREA #4 ENTRY 100' IN BY 1143 SPAD. 80' LONG 20' WIDE AND 6' THICK, TIMBERED OFF.
EE WAS SWINGING HEAD OF BOLTER, HEAD CAUGHT ON ROCK, STOPPED THE HEAD FOR A SECOND THEN TURNED LOOSE CAUSING THE HEAD TO COME TO FAR OVER AND CAUGHT HIS LEFT KNEE AGAINST THE HEAD OF THE BOLT ER.
OUTBY FACE 500' AT SPAD #1043 IN INTERSECTION 60'LONG, 39"THICK #6 HEADING WITH NO INTERFERENCE TO AIR. NO PLANS TO CLEAN IT UP TIMBERED OFF.
EE WAS GETTING A BOX OF GLUE OFF THE ROOF BOLTER AND HE GOT A CATCH OF PAIN IN HIS LEFT SIDE.
EE AND 4 CO-WORKERS WERE ROCKDUSTING AN UNDERGROUND BELT. A PIECE OF ROCK WAS SPOTTED WHICH WAS LOOSE AND OTHER TOLD EE TO PULL THE ROCK. HE WAS TRYING TO GET IN POSITION TO PULL THE ROCK AND HIT THE ROCK WITH HIS SHOULDER AND IT FELL.
EE WAS LOADING ROOF BOLTS INTO A SCOOP. HE SLIPPED OR LOST HIS FOOTING WHILE ON TOP OF THE SCOOP BUCKET. HE FELL INTO THE SIDE OF THE BUCKET AND HURT HIS BACK.
EE WAS ON A PERSONEL BUGGY IN RETURN. HE WAS IN REVERSE COMING OUT OF A PLACE HE WAS NOT LOOKING BEHIND HIM AND HIT A LOW PLACE AND JAMMED HIS NECK.
BAD TOP WAS TIMBERED OFF PRIOR TO ROCK FALL 2 BREAKS INBY SPAD #687.
EE RAISED THE LID ON MINER TO CHECK A WATER LEAKBUT DIDN'T RAISE THE LID HIGH ENOUGH AND THE LIDFELL ON HIS RIGHT HAND.
WAS PULLING PILLARS. SLIP IN TOP COVERED MINER UP.
SHOVELING BELT, BELT CAUGHT SHOVEL AND SLAMMED HIS HAND AGAINST THE ROOF.
INSTALLING A ROOF BOLT WHEN HIS GLOVE CAUGHT ON A ROOF BOLT. HE PUSHED THE ROTATION LEVER INSTEAD OF THE SUMP LEVER CAUSING HIS RIGHT HAND TO BETWISTED, BREAKING IT.
EE PICKED UP A 21 SC BUGGY TIRE. HE HAS BACK PAIN SINCE HE LIFTED THE TIRE.
HE WAS UNLOADING BELT STRUCTURE & HIT HIS ELBOW ON A RETURN ROLLER STAND INJURING ELBOW.
HURT KNEE BY CRAWLING AND TWISTING AROUND.
HE WAS HANGING THE MINER CABLE SLACK TO THE MINER WHEN HE GOT HIS THUMB CAUGHT BETWEEN THE ROPE AND CABLE.
OPERATING A 270 TRACTOR, HIT A ROUGH PLACE IN MINE FLOOR AND JARRED HIS BACK.
HE WAS CRAWLING AND TWISTED HIS KNEE.
WE WERE ON PILLARS SOMETIME ON THE MORNING OF 8/29/00. THE TOP CRACKED LETTING WATER RUN THROUGHTHE ROOF AND WATER FILLED A DIP ON SECTION AND SUBMERGED THE 2480 CONTINUOUS MINER.
EMPLOYEE WAS TRAMMING A 270 RAM CAR AND CAUGHT HIS RIGHT KNEE BETWEEN THE ROOF AND OPERATOR'S DECK.
EE WAS TIGHTENING THE CONVEYOR BELT WITH A SET OF RED DEVILS AND STRAINED HIS RIGHT SHOULDER AND ARM.
