Driver backed over the dump, flipping the truck. He was told not to back out but he did anyway. He caused approx. $30,000 cosmetic work to coal truck.
Employee was climbing into powder truck cab when he slipped and fell backwards. Employee tried to catch himself with his right hand. Broke bone in hand.

EE was welding on dozer blade. Dozer blade was blocked up and blade fell from blocks, hitting EE's right knee and injuring ankle.

Employee was traveling access road to get to the drill when the road broke off and turned over the truck he was traveling in.

EE was installing flooring on the 4th level of prep plant when he tripped and fell forward causing him to fall down to the 3rd level. He broke the lower right leg below the knee and cut his left wrist requiring 12 stitches.
