Employee put hand under the endpeller before it was secure. The endpeller struck his right index finger.

Fall from ladders

EE was working from extension ladder, when base of ladder slid back, causing EE and ladder to fall. EE received medical treatment on 2/22/11. this will be a lost time injury.

Struck by falling object

A cap from an oxygen cylinder fell off top floor of the plant fell 45' below and landed on the EE's right hand causing a laceration that required two stitches. The EE missed one day of work and returned to regular duty on his next sch. shift.

Employee was unstopping oversized rock chute. A big rock was lodge in chute, it dislodged, fell down the chute, bounced and struck employee in leg. Employee received medical treatment and lost 5 days.

Struck against a moving object

Driver backed over the dump, flipping the truck. He was told not to back out but he did anyway. He caused approx. $30,000 cosmetic work to coal truck.

EE was installing flooring on the 4th level of prep plant when he tripped and fell forward causing him to fall down to the 3rd level. He broke the lower right leg below the knee and cut his left wrist requiring 12 stitches.
