A power line contractor came by the maintenance planner's office and told him he noticed a gas pipeline leak near the auxillary stockpile area. MSHA was immediately called and the haul road near the leak was blocked to thru traffic. The pipeline was closed in and the pressure bled down. The haul road was re-opened at approx. 1:30 pm. Repairs completed at 6:00 pm 9/28/07.

Showing all 7O-ring ruptured in engine compartment of loader. Oil sprayed causing fire. Operator stopped loader / shut down using the e-stop. Fire subsided when operator exited cab. At the bottom of left stairway fire flashed burning employee's forearms and hands. Employee was treated and released from hospital and is on light duty. Machine damage was minor.
Unknown - employee said that it may have been from the 2 previous vehicle accidents he had been in.
EE was walking around front of truck to get lunchbox...foot slipped and EE felt pain in rt knee.
Employee was installing wear plate on dragline bucket. As he turned to get another piece of metal he tripped and landed on his side resulting in two cracked ribs.
Thr foreman walked into Rev Fr to check on crack being welded. Welder stepped from 3' ladder causing air gouger to fall to floor hitting trigger...sending debris from the floor into the air and into the foreman's left eye.
While getting in the cab of a buldozer, the employee was bitten by an insect on his left arm.
