EMPLOYEE WAS OPERATING TRUCK MOUNTED EXCAVATOR IN THE AREA OF THE CRUSHING PLANT. IT IS SUSPECTED THAT AN AIR OVER HYDRAULIC CONTROL DEVICE FAILED ON TRANSMISSION CAUSING THE VEHICLE TO GO OUT OF CONTROL AND CRASH INTO CRUSHER.

EE WAS DOING MAINTENANCE IN FINES PLANT. HE SLIPPED AND TWISTED HIS RIGHT FOOT. HE PULLED A LIGAMENT ON THE TOP OF HIS FOOT.

EE WAS GRINDING A POINT ON A CHISEL. A PIECE OF THE CHISEL GOT IN HIS EYE.

EE BENT OVER TO PICK UP TWO 55 GALLON EMPTY DRUMS, HE FELT A BURNING PAIN IN HIS LOWER BACK WHEN HE PICKED UP THE SECOND DRUM.

EMPLOYEE WAS LOOSENING STUCK CRUSHER IN PREPARATION PLANT WITH A PRY BAR. THE PRYBAR SLIPPED OUT OF THE CRUSHER GEAR AN DHIT HI IN THE FOREHEAD.

EE WAS REMOVING OIL FILTER BRACKET FROM LOADER IN AWKWARD POSITION FOR SEVERAL HOURS & AGGRAVATED PREVIOUS DISC INJURY.

EMPLOYEE WAS OPENING A GATE IN THE MORNING. HE DROPPED THE KEY,LEANED TO PICK IT UP.

EE WAS USING A SHOVEL AND POKING BAR IN BREAKER TO BREAK UP FROZEN LUMPS OF COAL.
