At approximately 4:40pm the service hoist on the Camp Creek portal went down due to a contactor fault. At 5:48 pm the contactor was repaired and the hoist was put back in operations.

While trying to put the pin back in the cat track on J-main feeder with a 4lb hammer the employee hit the top of left hand. Employee did not start missing time from this injury until November 16, 2018.

Employee was pulling on a chain when EE felt a burning pain in abdomen area.

While lowering the slope car to the bottom the car came to a stop and caused the hoist rope to backlash on the spool.

Employee was removing bad belt frame when EE mashed left index and middle finger.

Employee was closing the gate to the elevator when EE's little finger on the left hand got caught between the gate roller and the gate.

Employee was removing a oil filter and felt sharp pain in right shoulder. Employee did not start missing time from this injury until 10/19/2018.

Employee was cutting conveyor belt with a utility knife when the knife slipped cutting the employee above right knee.

Employee was walking down the hill at cross cut 4 on H-2 belt when foot slipped causing employee to twist left knee. Employee did not start missing time from this injury until 12/10/2018.

Employee was walking next to belt line when EE slipped on a hose and as EE tried to stop the fall, employee strained left shoulder.

While walking in Bath house on concrete floor at the start of employee shift the employee stepped and pushed off with right foot felt pop in right heel. Employee did not start missing time from this injury until 10/03/18.

On Thursday September 27, 2018 at approximately 6:20pm a roof fall occurred on the J-1 section in the number 1 entry on the J-1 longwall set up. The roof fall is approximately 30' long by 21' wide by 9' high.

The elevator at the portal went down at 4:56pm due to a faulty gate switch. The switch was replaced and the elevator was put back in service at 7:15pm.

Employee was moving power box sled when employee's right foot slipped off toe of shield causing employee to twist right ankle.

While installing a BFR door to the longwall stage loader a water jack became dislodged striking an employee across the lower legs. The water jack was used to keep tension to the BFR door in order to connect the ram jack pin to the door.

Reportable only due to sutures. Employee was walking up on overcast when ee cut left middle finger on some tin on the overcast.

During the daily service check of the elevator the computer program faulted out at 5:25am and could not be reset.

At 9:42 am the hoist at Camp Creek portal went down due to a control stop fault that showed up on the computer screen and would not reset. The company got another computer and reset the fault at 10:10 am.

Employee was walking back to loader on J-1 section between cross cut 1 and 2 when a rib rolled out hitting the employee on the right ankle.

While working on 5710 loader, the employee stumbled over a timber that was laying on the ground as ee was walking around the loader and twisted right ankle.

Reportable only due to sutures. Employee was cutting a timber with a hand saw when the saw slipped causing employee to cut left thumb.

Employee was stepping down a ladder the North main #2 roadway at cross cut 24 when left foot slipped on a rock causing employee to twist left ankle.

Reportable only due to sutures. Employee was removing small rocks from the hand rail on the H-2 long wall face when a rock fell from between the shields striking employee index an middle fingers on right hand.

Employee was operating a Jeep personnel carrier (2787) on the west main left roadway at cross cut 70 when the Jeep hit the right rib causing it to turn over on it side.

Reportable only due to sutures. Employee was hanging miner cable when the cable fell causing the employee right arm to hit against the side of the bucket on the 3.5 loader resulting in laceration to right foreman.

At approximately 10:30 am on June 20th, 2018 the camp creek hoist went down due to the fault code to the over travel switch. Repairs was made and the hoist was put back in operations at 11:40 am on June 20th, 2018.

During the 5 year required test of the Shoal Creek elevator the people conducting the test heard a noise. They tested the elevator by running it up and down the shaft and never could find the source of the noise. That night around 1:50am, we think the electrician found the source of the noise. A metal plate got bent during the test and it started rubbing the guide wheel.

Employee was operating a Cat 651 scraper SN/67K751 at the start of the shift. As the employee put the transmission in gear it jerked causing employee to strain their neck.

Employee was walking along the H-2 belt entry at cross cut 23 when EE hyper-extended EE's right knee. Employee did not start missing time from this injury until 6-14-18.

Employee was stepping out of a man trip when the employee twisted left knee. Employee did not start missing time from this injury until July 28, 2018.

Employee mashed left middle finger as they were loading a re-lay bar on the shearer to bring to the head gate end of the long wall face.

The elevator at the Shoal Creek portal went down on 05/06/2018 at 8:20 pm due to a faulty door switch contactor. The contractor was replaced and the elevator was put back in service on 05/07/2018.

Employee was traveling thru man door in brattice when the man door slammed on left hand and wrist area.

Employee stated that EE was carrying material and going in and out of man door and at some time during the shift EE strained left rib area. Employee did not start missing time until 05/01/2018.

Employee was walking down west main belt at crosscut 93 when a rib rolled out hitting employee in the right ankle.

Employee was walking when employee slipped and twisted right knee. Employee did not start missing time from this injury until 05/03/2018.

Employee was lifting an impact head out from a container and as EE turned to put it on the lift EE felt pain in lower back. Stated missing time from the injury on 04-07-2018.

Employee was helping set timbers in an area where a rib had rolled out. After cleaning up the area and loose rib material removed. The timbers where set and secured as required by the roof control plan. The rib rolled out again breaking one of the timbers and causing the timber to hit the employee in the upper back and shoulder area.

Employee was removing a rock from west main #3 belt line and as they were attempting to place rock in bucket they felt pain in lower back.

Reportable only due to sutures. Employee was cleaning in the raw coal tunnel on the surface when ee hit head on a low metal I-beam causing a laceration above the left eye.

Reportable only due to two sutures. Employee was getting off the small elevator at the prep-plant and went to shut the elevator door and shut the door on right forearm.

A roof fall was discovered at approximately 8:15 am on the morning of February 15, 2018 in the north main return # one entry between cross cuts 52 and 53. The roof fall measured approximately 50 long by 20 feet wide and approximately 5 to 6 feet in height.

Employee was moving water pump when employee strained right shoulder. Employee did not start missing time from this injury until 03/12/2018.

Reportable due to sutures. Employee was trimming belt when the knife EE was using slipped cutting employee above right knee.

Employee was tightening a fitting on a hydraulic hose when they felt a pop in left elbow.

Employee was making EE's examination at the camp creek hoist when EE slipped causing EE to twisted and hit EE's left knee. Employee did not start missing time due to this injury until 02/12/2018.

An ignition occurred on the H-3 section at approximately 5:55 pm while mining the #2 face.

Employee was lifting a bottom V-belt roller and felt a pop in right shoulder.

Employee was washing down at the slope tail roller when they slipped and twisted left knee. Employee did not start missing time from this injury until February 28, 2018.

Employee was cleaning muck and coal fines at the west main belt discharge area when ee bumped head on a pipe causing ee to lose balance hitting right forearm on belt structure.
