At approximately 4:40pm the service hoist on the Camp Creek portal went down due to a contactor fault. At 5:48 pm the contactor was repaired and the hoist was put back in operations.

At approximately 10:30 am on June 20th, 2018 the camp creek hoist went down due to the fault code to the over travel switch. Repairs was made and the hoist was put back in operations at 11:40 am on June 20th, 2018.

During the 5 year required test of the Shoal Creek elevator the people conducting the test heard a noise. They tested the elevator by running it up and down the shaft and never could find the source of the noise. That night around 1:50am, we think the electrician found the source of the noise. A metal plate got bent during the test and it started rubbing the guide wheel.

Employee was helping set timbers in an area where a rib had rolled out. After cleaning up the area and loose rib material removed. The timbers where set and secured as required by the roof control plan. The rib rolled out again breaking one of the timbers and causing the timber to hit the employee in the upper back and shoulder area.
