Employee was opening truck hood. The hood would not open so the employee stood on the bumper of the truck to get a better pull and when the hood opened it came down on the employee feet before he could get out of the way.

Bottom step broke when employee was descending causing him to fall to ground with a jolt. He went to dr. for ex rays-given pain pills- no/broken bones.
Employee was using a cutting torch to do repair work and a piece of metal went under his safety glasses and stuck in his eye. Went to Dr. and had it removed and given medicine.
Employee was measuring auger span with metal tape measure & the side of the tape measure cut his right index finger. Went to hospital, 6 stitches.
EE as operating atriculator truck to be loaded. Another EE was loading is truck with a 988 loader, when the bucket hit the truck, causing a jolt to EE. His back was jolted, causing his right leg to have numbness.
Employee was getting out of truck to get truck ticket & twisted left foot as he was returning to truck.
Employee was changing drill bit and it fell on his foot.
EE was on the highway to the strip job to get a load of coal and had to turn around, due to a piece of state equipment in the road. He got out of his truck and stepped into a 3-4' ditch, hurting his back. Went to ER for X-rays, they were clear.
Employee was in office weighing trucks. She stood up to leave and somehow tripped and fell on floor. She hit her left shoulder and strained her left foot. Went to E.R. They said sprain of left ankle, x-rays on shoulder & left ankle - do not know about shoulder at this time.
EE was watching automatic track torch welder run on miner beam. Sparks came off of torch when welding and caught EE's pants on fire. He then proceeded to pat out flames with hands.
Employee was walking down miner beam toward the front of machine & it was raining/muddy - he lost his footing & slipped/fell to ground hitting left side & back. Went to Dr. for x-rays - possible bruising.
Was moving rock and material with dozer and dozer blade slipped off edge of rock causing dozer to jolt.
Cutter Head and beams stuck underground.
Employee was lifting auger end and it slipped out of his hand and caught it between auger and auger beam. No broken bones. ER put on splint and glued laceraction together - no stitches. Tetnus shot.
Employee was using a winch type tool to pull cutter head together with highwall mine. It slipped rolling against his knee & highwall miner frame. X-rays - no broken bones.
Employee was helping another man lift the miner jack and it slipped from his hand & fell on right hand causing fingers to get hurt.
Employee possibly had a stroke while working. Was sitting in cab of highwall miner when another employee noticed slurred speech.
Employee was walking on catwalk by rear powerhead - slipped and fell against chain take-up. (Insecure footing.)
Was cutting trees & tree split, man slipped on icy ground and fell. Then, tree jumpede off stump and onto man's left leg. This was a contract tree cutter.
He was pulling on a belt while working on a pump and his rt. ring finger got caught between the belt & the pulley. Went to hospital - x-rayed & it was broken on the end.
Changing oil filters on equipment (?) Will have operation after Christmas. Employee stated he didn't know how or when this happens.
Employee was climbing onto equipment by ladder. As he got to the top, he lost his footing and slipped down the ladder, catching his right hand on the handrail. Laceration to right middle finger and some scrapes on stomach.
Man was changing tire on coal trailer while another mechanic was working on a bar that goes on the automatic tarp cover. The bar sprung back striking man on lower rt. arm. Was taken to E.R. and sent to an orthopedic surgeon to set broken bones.
Foreman was getting truck tickets out of cab of loader. He was descending ladder and misjudged the last step. He was still holding onto ladder and swung body into handles, bruising ribs, Went to ER on 4/15/06 for X-rays - only bruising.
ee was trying to get ball off of trailer hitch witha pry bar-strained back
EE was getting onto dozer and twisted his back. He went to the Dr. on 4/8/2006. Dr. believes it to be a pulled muscle. EE came back to work on 4/7/2006, but is returning to Dr. for reoccuring pain.
EMPLOYEE WAS PUTTING IN A SPLICE AND THE KNIFE SLIPPED, CUTTING HIS RIGHT FOREARM. HE WENT TO THE HOSPITAL AND THEY CLEANED IT, PUT 2 STAPLES IN IT, AND ADVISED TO TAKE 1 DAY OFF.
EMPLOYEE WAS WALKING DOWN STAIRS OF PARTS WAREHOUSE AND SLIPPED ON ICE AND MUD, TURNING RIGHT WRIST AND PUTTING LACERATION ON LEFT FOREARM. WENT TO ER FOR X-RAY (NO BROKEN BONES).
Man was walking in pit area and stepped into a hole full of water - twisted right ankle. Foreman took him to hospital for x-ray. Right ankle was sprained.
Was cutting piece of metal w/hand torch-piece of spark went under glasses into rt eye. Went to Dr. and was given eye drops.
Was going to lift metal band that connects drainage pipes. Hurt center of back dr said might be pinched nerve.
Employee was working inside back of welding truck. He slipped and fell on welder in back of truck hitting left side of ribs (snow and sleet conditions). He went to hospital for x-rays: only bruised ribs.
Employee was cleaning around belt roller at sampling unit with a shovel. The shovel hit the roller & caught ee's left index finger between shovel & roller frame. EE went to the hospital, injury required 5 stitches and he also received a tetnus shot.
Employee #1 was driving service truck and met another truck on roadway. Employee #1 backed up to let the second truck pass and backed into a ditch, turning service truck partially on its side.
STEAMING HIGHWALL MINER, & SLIPPED TWISTING ANKLE
Employee was getting dirt off loader tires and was going back upon the equipment-hit the top of his head on the mirror frame.
Employee was using steam ginny to clean off highwall miner - slipped on wet surface injurying ribs & rt shoulder. Went to Dr. Has cracked ribs & pulled muscle in shoulder.
EE SLIPPED & FELL HITTING RT LEG & LEFT ELBOW. WENT TO DR. ON 7/5/05 DR SENT HIM FOR XRAY
Climbing off dozer while doing repair work, he lost his footing and slipped causing sprain or muscle injury.
EE had steamed off dozer. While stepping down off dozer he slipped and hit right shin bone (lower leg). Cut skin which did require 16 stitches. Leg was not broken.
While employee was climbing down from rock truck, he slipped on the last step down and turned his ankle, breaking it.
EMPLOYEE WAS HELPING HAUL MOTOR, HE WAS TRYING TO GUIDE CHAIN, MOTOR FLIPPED AND CHAIN CAUGHT EE'S 3 FINGERS: MIDDLE, RING & PINKY. MIDDLE FINGER REQUIRED SURGERY.
EE WAS CHECKING RAW COAL CRUSHER FOR MATERIAL BUILDUP, AS HE STARTED DOWN OFF PLATFORM, FOOTING SLIPPED & HE HIT CORNER OF METAL COVER WITH RIGHT ARM JUST BELOW ELBOW. LACERATION REQUIRED 5 STITCHES INNER & OUTER
EMPLOYEE STATED HE WAS PUTTING PIN IN A BRACE THAT HAD COME LOOSE ON HIGHWALL MINER CONVEYOR. LOST BALANCE AND FELL, LANDING ON RIGHT HIP & LOWER BACK. INJURY TO RIGHT HIP AREA & LOWER BACK - POSSIBLE FRACTURE.
EE WAS PULLING 6" WATER PUMP HOSE, AND PULLED MUSCLE IN LOWER BACK. CONTINUED TO WORK UP TO 4-26-04 TAKING THERAPY. DR ADVISED TO TAKE OFF 4 OR 5 DAYS TO SEE IF IT HELPED BACK.
EMPLOYEE WAS LOADING SCRAP METAL BY HAND, FELT PAIN IN LOWER BACK AND LEFT LEG, HE CONTINUED TO WORK UP TO 8-4-03, PAIN GOT TOO SEVERE, HE WENT TO DR, MAY HAVE DISC BULGED.
EMPLOYEE WAS HELPING WORK ON EXCAVATOR, LIFTED BATTERY COVERS OFF AND PUT THEM IN SERVICE TRUCK, FELT PAIN IN LOWER BACK. CHECKED BY LOCAL DR, GOING TO DO MRI, AS OF NOW MUSCLE STRAIN.
EMPLOYEE WAS HELPING MOVE ROCK AWAY FROM MINER, ROCK CAME OFF BANK & HIT EMPLOYEE ON RIGHT LEG JUST ABOVE KNEE, CUT LEG REQUIRING 2 STITCHES, AND DAMAGED LIGMENTS & TENDONS. HOSPITAL STAY FOR X-RAYS, MRI, ETC & THERAPY.
EMPLOYEE WAS ATTEMPTING TO MOVE A WATER PUMP - FELT DISCOMFORT IN ABDOMEN AROUND NAVEL. REASON FOR LATE REPORT DR SEEN 6-17-03. THEY SAID HERNIA & SURGERY 7-2-03. NO LOST TIME TO DATE.
