Unintentional roof fall in No. 4 entry on No. 2 belt (secondary escape way) at survey station No. 463. Sides of fall were slick sided and appeared to be horseback in roof. The mine was in a non-producing status and under going a re-opening inspection at time of fall.
Rock fall in intersection at Brk #21.
EE was hanging door. Door came loose and knocked ladder from under ee with ee falling to floor.
EE was moving the miner from the #7 to the #6 entry. The miner went over a hump in the bottom, this caused the back of the miner to raise into the top.
While tramming shuttle car from miner to dump, shuttle car dropped in a hole. The deck of car kicked up a rock, hitting EE in the eye.
The continuous miner on the 002 section was cutting the 2 left crosscut into the #1 heading 120 ft inby SS 863 when it cut into old works. A hole app 2 ft x 4 ft intersected into the old works. No low O2, Methane or water was encountered during the hole through. Personnel was removed from the mines with no injuries.
Roof fall occurred in an area where evidence of a low-angled slicker sided condition was present in the mine roof within a transition area evident both inby and outby affected area.
While loading roof bolt supplies into scoop, the scoop operator dropped scoop bucket on employee's right foot.
While mining in the # 5 entry of the 001 section, cut into old works. The size of the cut through approx. 2' X 4'. The mine is blowing ventilation therefore no ventilation issues with cut through.
Co-worker was driving pin out of matt chain while worker was holding pin. Hammer glanced off pin striking worker in mouth.
Found on re-open inspection.
Employee was helping install a D10R center equalizer bar pin along with two other field technicians. The pin was struck awkwardly by another technician when a splinter of metal came off of the side and struck employee in the groin area of his leg.
Fire was set and belt burned.
The miner operator while operating a continuous miner was traped between the rib and continuous miner resulting in a bruised lung.
While striking idler roller with hammer, a piece of steel broke off striking employee in forearm.
Conducting a pre-shift inspection, the victim was struck on the back by a rock that fell between bolts, injuring ribs and right side of body. Rock measured 3" thick and 12" long.
VICTIM AND TWO EMPLOYEES WHERE IN THE PROCESS OF CHANGING OUT SURFACE HIGH VOLTAGE TRANSFORMERS. DURING THE PROCESS THE VICTIM CAME INTO CONTACT WITH THE HIGH VOLTAGE CURRENT, RESULTING IN INJURIES INVOLVING BURNS, CUTS AND BRUISES. VICTIM LIVED UNTIL 10/29/04 AT 3:00 P.M.
EMPLOYEE WAS MOVING CURTAIN, CABLE REEL CAME ON THE ROOF BOLTER & A ROOF BOLT CAME OFF OF PINNER STRIKING EE IN FACE/HEAD.
EMPLOYEE WAS REACHING A HYDRAULIC HOSE TO ANOTHER EMPLOYEE WHEN HE SLIPPED AND FELL ABOUT 7 FEET ON HIS RIGHT WRIST.
ENDLOADER WAS COMING FROM COAL STOCKPILE BACK DOWN TO RAIL SIDING WITH LOADER BUK RAISED BLOCKING HIS VISION AND RAN INTO LOADED COAL TRUCK. TRUCK DRIVER SAID HE SEEN HIM COMIONG AND STARTED S OUNDING AIR HORN ON COAL TRUCK. LOADER NEVER STOPPED, SO COAL TRUCK DRIVER STOPPED AND LOADER KEPT COMING AND RAN INTO THE LOADED COAL TRUCK.
FELL ON UNLEVEL GROUND TWISTING KNEE.
JUMPED OFF LOADER LANDING ON RIGHT FOOT.
