Unintentional roof fall in No. 4 entry on No. 2 belt (secondary escape way) at survey station No. 463. Sides of fall were slick sided and appeared to be horseback in roof. The mine was in a non-producing status and under going a re-opening inspection at time of fall.

EE was hanging door. Door came loose and knocked ladder from under ee with ee falling to floor.

EE was moving the miner from the #7 to the #6 entry. The miner went over a hump in the bottom, this caused the back of the miner to raise into the top.

While tramming shuttle car from miner to dump, shuttle car dropped in a hole. The deck of car kicked up a rock, hitting EE in the eye.

The continuous miner on the 002 section was cutting the 2 left crosscut into the #1 heading 120 ft inby SS 863 when it cut into old works. A hole app 2 ft x 4 ft intersected into the old works. No low O2, Methane or water was encountered during the hole through. Personnel was removed from the mines with no injuries.

Roof fall occurred in an area where evidence of a low-angled slicker sided condition was present in the mine roof within a transition area evident both inby and outby affected area.

Employee was helping install a D10R center equalizer bar pin along with two other field technicians. The pin was struck awkwardly by another technician when a splinter of metal came off of the side and struck employee in the groin area of his leg.

Conducting a pre-shift inspection, the victim was struck on the back by a rock that fell between bolts, injuring ribs and right side of body. Rock measured 3" thick and 12" long.

TRUCK DRIVER SAID HE SEEN HIM COMIONG AND STARTED S OUNDING AIR HORN ON COAL TRUCK. LOADER NEVER STOPPED, SO COAL TRUCK DRIVER STOPPED AND LOADER KEPT COMING AND RAN INTO THE LOADED COAL TRUCK.
