WHILE FUELING UP DOZER, TAKING FUEL NOZZLE OFF WHEN FUELING WAS COMPLETE, FUEL SHUT OFF BUT THEVALVE ON DOZER DID NOT CUT OFF. THIS CAUSED FUELTO COME OUT OF THE DOZER TANK FUEL UP RECEPTICAL WITH FORCE HITTING EE IN THE FACE.

DRIVER WAS ON TOP OF THE TRUCK BED REMOVING SLATE ROCK FROM THE COAL. SOMETHING HAPPENED CAUSINGTHE DRIVER TO FALL FROM THE TRUCK LANDING ON HISHEAD.

EMPLOYEE WAS WORKING ON 785B AND NEEDED TO CLIMBLADDER TO GAIN ACCES TO THE BRAKE ACCUMULATOR. THE LADDER ON HIS TRUCK WAS A 6' LADDER AND WAS TOO SHORT. HE GOT THE CUSTOMER'S 10' WOODEN LADD ER. WHILE CLIMBING THE LADDER, IT BROKE CAUSING EMPLOYEE TO FALL.
