DRIVER WAS ON TOP OF THE TRUCK BED REMOVING SLATE ROCK FROM THE COAL. SOMETHING HAPPENED CAUSINGTHE DRIVER TO FALL FROM THE TRUCK LANDING ON HISHEAD.

EE was driving a Mack Coal Truck hauling coal from the surface mine down the haulage travel way when EE got it out of gear and could not get it back in gear and put the Mack Coal Truck in the ditch between the #3 and #4 marker on the haulage travel way.

EE was installing a thrown track on a D10R when a job mechanic struck metal on metal with a sledge hammer resulting in a metal shard puncturing EE's chest.

EE arrived on the job at approx. 5""50 am. EE told the foreman his leg was hurting & he could not run the dozer. The foreman told ee to go to the parking lot & when he got the job going he would be back. When the foreman returned he found ee unconscious. He started CPR & continued it until the rescues squad arrived. EE was pronounced dead at Hospital

Employee stated he was in the porta trailer. He was attempting to exit the trailer when he slipped and fell. It was raining, his boots were wet & muddy. Lost his balance.

The hauler was on a slick incline and could not pull up the hill the hauler driver requested that the dozer give him assistance, the hauler tires were spinning and when the tires got traction, the hauler jerked, causing it to jerk the operator, hurting his back.

Operator was tramming a dozer to the pit and alleges he pulled a muscle in his lower back. This was not reported until the 27th.

Employee was dismounting a D-10 dozer. His right foot slipped off the push beam causing him to fall to the ground landing on his right knee and leg.

EE was putting chains on a motor grader using a hammer. While striking the chain roll pin, a piece of the hammer broke off and struck the EE in the bottom of the right eyelid. A sliver of metal was removed by a doctor; glue was used to seal hole in right eye-ball.

Employee was lifting a heavy component using his boom, when the chain he was using slipped off mashing his right ring finger. Required stitches.

Employee was climbing down from a hauler. When he stepped onto the ground his foot landed on a can of oil, causing him to twist his knee. Did not seek medical until 10-21-08. Continued to work with no missed time until 4-9-09. He had surgery on his right knee.

Employee was using an electric grinder when it slipped out of his hand and hit his leg. Grinding disc was still moving which caused an 1 1/2"" cut on leg.

Employee was operating a hauler, it caught on fire, he had to exit the hauler, went down three steps and then jumped to excape the flame spraining his left ankle.

EE was reaching over the frame of a 994 loader to check the transmission oil and slipped, felt pain and some discomfort in his lower back. EE continued to work and did not seek medical treatment attention until 3-10-08 and is off from work now.

Employee was using a fire extinguisher to put out a small fire. Hydraulic hose ruptured and ignited. He inhaled smoke and fumes.

Employee was using an impact gun with a wobble head socket. The socket broke and a piece of metal from the socket cut the palm of his left hand where the thumb joins the hand.

Employee was using 1 1/2"" cable to pull dozer and when finished, attempted to remove cable from the dozer and in the process the clevis struck ee in upper left corner of left eye causing a laceration requiring 8 stitches.

Employee was preparing to raise access ladder on shovel, he bent over to pull the release pin, felt a pop in his lower back.

EMPLOYEE WAS GETTING OUT OF FUEL TRUCK, GRABBED THE HANDRAIL WITH ONE HAND & THE SEAT HANDLE WITH THE OTHER. WHEN HE TOOK ALL HIS WEIGHT OFF OF THE SEAT, THE SEAT WENT UP & CUT HIS FINGER WHICH REQUIRED SUTURES.

Employee and another co-worker were repairing an rh200 shovel and were using a battery powered light. the battery part of the light was knocked to the ground and in doing so jerked the light out of co workers hand and knocked injured ee in the mouth breaking a tooth.

EE was pushing in the shot with a d11, a rock rolled in behind him and when he backed up he hit the rock, causing a hard jolt. He felt something pop in his neck.

Employee had just taken a chain out of the side compartment of the boom truck and started to hook the chain on the boom hook, and as he turned he felt a severe pain in his back.

EE WAS AUGERING WHEN ROCKS STARTED FALLING FROM THE TOP OF THE HIGHWALL ONTO THE AUGER. THE METAL CANOPY WAS BROKEN THRU AND THE CAB SUSTAINED BROKEN GLASS. THE EE WAS CUT ON HIS RIGHT INDEX F INGER THIS REQUIRED 6 STITCHES.

WHILE FUELING UP DOZER, TAKING FUEL NOZZLE OFF WHEN FUELING WAS COMPLETE, FUEL SHUT OFF BUT THEVALVE ON DOZER DID NOT CUT OFF. THIS CAUSED FUELTO COME OUT OF THE DOZER TANK FUEL UP RECEPTICAL WITH FORCE HITTING EE IN THE FACE.
