Getting off equipment, jumped down and twisted ankle. Ankle was sprained.
EE was lifting a sieve screen on #5 shaker when EE pulled rib area. No conditions other than lifting.
Shoulder injury due to sledge hammer.
Employee was handling a section of metal tubing. The tubing was thought to be in a stable position. The tubing accidentally rolled onto his foot.

WHILE HAMMERING A STEEL PIN INTO THE TRACK OF AN EXCAVATOR WITH A SLEDGE HAMMER, A SMALL PIECE OF THE PIN BROKE LOOSE AND STRUCK EE IN THE RIGHT LOWER LEG CAUSING A PUNCTURE WOUND.
