While moving a cart weighing approx. 200 lbs. the employee backed into a pallet and pinched their shin between the cart and pallet.

Getting down off truck slipped and hit right hand off truck

Getting off equipment, jumped down and twisted ankle. Ankle was sprained.

EE was lifting a sieve screen on #5 shaker when EE pulled rib area. No conditions other than lifting.

While removing an old feed chute, plant operator had his right hand pinched between the chute and a 4"" water line, lacerating his right pinky finger.

Working on belt press, while getting down, EE jumped 2'. The cable caught him in the groin area and somehow jarred his right shoulder. He grabbed the rail with his left hand.

Slipped with utility knife while stripping cable.

WHILE STEPPING OF LOADING DOCK, EMPLOYEE LANDED ON A PIECE OF CONCRETE AND SPRAINED LEFT ANKLE.

GRINDING ON METAL PLATE WITH HAND GRINDER. GRINDING WHEEL CAUGHT ON METAL PLATE AND GRINDER KICKED BACK INTO LEFT ARM.
