UNCHOKING CONE CRUSHER WITH PRYBAR WHILE CRUSHER WAS RUNNING. PRYBAR STRUCK CONE CAUSING IT TO TWIST. INJURED CONTRACT EMPLOYEE SUFFERED TWO LACERATIONS TO RIGHT HAND. INJURED CONTRACT EMPLOYEE WAS NOT WEARING GLOVES AT TIME OF INJURY.

Rainy weather, Mud, stepping off machine and foot rolled on stone spraining and Ankle.

Mechanic was using come along to hold tension on a steel bar as they heated it with a torch , the weld broke on bar and it came back and hit them on the left leg.

Federal Mine inspector arrived on site, and as inspector was driving up haul road apparently was experiencing severe health issues, which was a result of inspectors passing of natural causes.

Operator was assisting in changing screens at wash plant. Using a pry bar to pry the bar slipped pinching operator's finger against screen

Driver was exiting the truck. As EE was backing down the steps EE missed the last one and fell backwards landing on left hand

Walking through the brush. Slipped and fell putting hand down to catch fall and run a small stick into right hand.

Employee was moving a barrel of caustic soda and it splashed hitting employee in left eye

Operator was exiting machine and while going down ladder, EE slipped and fell and caught self with right arm and shoulder.

Installing snake fence on perimeter of mine site.

Bent down to retrieve a piece of angle iron and hit right side of face on protruding piece of steel

Operator was entering cab of machine, he had his right hand on door frame, wind blew door shut catching his right index finger between door and frame

The Employee had dismounted his front end loader and was standing next to the loader when he slipped. The slip and fall accident was due to the Ice that he was standing on next to the loader

Working from an elevated position using cutting torches when a spark came off striking the miner in right ear. The hot spark entered the ear and damaged the ear drum. No loss time took place. On 10/13/2014 the miner went back to the Doctor and received an antibiotic cream and surgery is now going to be required to fix the damage to the ear.

Piece of Metal came off of a pin and went into the Right eye of the Employee

Cut to Left Hand due to piece of metal cutting hand. Required stitches to cut

Equipment Operator was conducting his pre-shift inspection when he slipped on the push arm on the dozer he was going to operate and fell hitting his ribs on the push arm of the dozer. Weather conditions contributed to the accident.

Burned on chest and arms from steam from a 4" water pump

Employee was getting off of the 992 Loader when his right foot turned over tearing his Achilles tendon

The miner placed his right thumb in a pinch point between the tilt cylinder and idler on a CAT front end loader.

Fell while climbing off of loader. Hurt wrist when landing.

While scaling the highwall, a rock came loose and crushed the cab of the excavator. The operator's only injury was a bruised knee.

Dropped piece of steel on foot.

Slip & fall on ice.

Sprained ankle when getting off dozer. Tripped on rock and lost footing.

Employee was stepping off machine and twisted ankle

BLASTER WAS BREAKING UP FROZEN STEMMING WITH A PICK. HE SLIPPED ON ICE AND TWISTED LEFT KNEE.

WHILE ADJUSTING BOLTS ON MAIN GENERATOR, 1 1/4" SOCKET CAME OFF AIR IMPACT WRENCH AND STRUCK EE IN THE FRONT RT. UPPER SIDES OF HIS MOUTH (BOLT BROKE). THIS CAUSED THE FRONT TOOTH TO BREAK ABOVE THE GUM LINES. THE DENTIST SAID THE TOOTH HAS TO BE EXTRACTED. THE DENTAL BRIDGE IS BROKENAND NEEDS FITTED. MOUTH IS ALSO BADLY BRUISED & INFECTED.

IT WAS SHIFT CHANGE, EMPLOYEE FUELED HIS MACHINEAND WAS ABOUT TO PUT ENGINE OIL IN THE DOZER. HEWAS WALKING FROM THE OIL TANK TO HIS DOZER WHEN HE SLIPPED ON A PATCH OF ICE AND FELL TO THE GROUND.

EE WAS CLIMBING INTO THE FRONT DRILL CAB AS OPERATOR WAS LOWERING THE LEVEL JACKS. EE WAS STEPPING ON THE FRONT TIRE INSTEAD OF THE STEP PROVIDED BY THE MANUFACTURE. AS THE WEIGHT WAS PUT ON T HE TRUCK SUSPENSION THE SPACE BETWEEN THE STEP AND THE TIRE GREW SMALLER FINALLY PINCHING HIS FOOT BETWEEN THE TIRE AND THE STEP.
