Showing all 39While installing a missing nut for a bolt on a pump, the base plate came loose and fell approximately 8 inches striking EE on the right wrist fracturing it.
Operator closed cab door of loader and right index finger was stuck between the door and the door frame, causing a partial amputation of the index finger.
EE was cutting away flooring when EE cut the flooring EE was standing on. This in turn caused EE to fall from an unknown height to the ground below EE.
Employee was stepping down off the back of a service truck when employee slipped and fell to the ground. Causing a wrist fracture. It was down pouring rain at 7:00am when the incident occurred.
Employee was moving suction hose to a deeper position when a metal screen slipped out of EE's hands and the sharp end cut through EE's work gloves and into EE's left index finger. The laceration required stitches and a tetanus shot.
Removed access panel and placed it on the tail pulley guard on the back of conveyor. As they were gaining access to chute the access panel slipped from its position and punctured a finger requiring stitches.
Employee was removing a blow bar on the crusher using a hammer and swung the hammer to strike the wedge to dislodge it, but missed the striking target and got finger between the hammer handle and the door of the crusher. Causing right index finger to bleed a little and swell a little. Was given general first aid at onset, but later went for treatment.
Subject located on mine property deceased from apparent suicide. Authorities notified of abandoned vehicle day prior and came back to search area for missing/wanted man. Found subject at 14:41 and req coroner at 14:44. No mine accident or injury, just trespasser.
Changing pillar block bearing, chain come-along unlatched and shaft landed on my finger and bolt.
Was repairing belts on Lower Dust Collector when hand slipped between belt sheave, cut right small finger.
Employee climbed up the right side of the ladder and slipped on the ladder falling to the ground on knee. Twisted and sprained right knee.
Driver backed to the berm of the dump site and did not stop. 85 ton rock truck he was driving pushed through the berm. Truck rolled down the embankment and the driver jumped from the truck.

Employee was cleaning auger hole and some piece of top was going up conveyor. He went to the conveyor to throw the pieces off. A piece rolled down and hit his finger between a rock and the paddle on the conveyor.
Employee was hitting side of dryer with a hammer when hot material came out of thermal coal dryer next to him.
Was loading hoses and water pump into bucket of excavator, when the bucket moved and struck employee in the chest.
Was using impact wrench and wearing safety glasses, when object got into eye
Had been grinding in the shop, went to restroom to rinse off face and washed object into eye
Reached into feeder to clean out coal dust, got fingers stuck in paddles. Cut forefinger and middle finger on left hand.
Impact socket blew apart striking employee in mouth and face. Employee lost a tooth, has a fracture bone in face and a fractured nose
Was working on dozer changing roller cup. Had raised boogie with jack, jack slipped causing boogie to come down pushing down impact wrench and injuring wrist.
Was cutting rubber hose with utility knife, knife slipped and employee cut his hand.
Contract truck driver stepped on a loose piece of decking and twisted his knee with a minor abrasion
Employee had a heart attack and died
Was spray painting numbers on wall. Was facing highwall when some rocks fell off the wall landing on ee-a couple heads higher than ee-rocks did not fall from top.
CLEANING TRACKS ON DOZER - CHUNK OF ICE / MUD HIT FINGER.
PULLED UP TO DOZER TO FUEL - HE STEPPED OUT OF TRUCK & WALKED BACK TO GET FUEL HOSE - PART WAY BACK A ROCK HIT HIM IN THE CALF.
Stepped out of machine and fell. No medical treatment, Physician gave him three days off. Had a contusion to the left stomach wall.

WAS TRYING TO MOVE A 3" WATER PUMP BY HIMSELF. WHEN HE PULLED UP TO LIFT, INJURED BACK.
PULLING AND SETTING UP 4" HOSE & PUMP TO DEWATE SLURRY PONDS FELT PAIN & DISCOMFORT IN RIGHT GROIN AREA.
ROCK FLEW OUT FROM UNDER TIRE OF 150 TON TRUCK AND HIT LT. LEG IN CALF.
EE WAS POWER WASHING A BULLDOZER. TURNED AND SLIPPED AND HURT HIS SHOULDER (LEFT) WHEN HE SLIPPED.
MECHANIC AND EE WERE MOVING HEAVY OBJECT ON DOLLY. DOLLY WHEELS STRECH. PULLING DOLLY OUT WHEELS WENT TOO FAR AND DROPPED OFF STIR EE TRIED TO STOP AND PULLED MUSCLE IN RIGHT ARM.
