Employee states he opened the door on the truck and stepped out, feeling a sharp stabbing pain in his lower back. EE obtained medical treatment and placed on modified duty 3/14/14. The EE has not lost time due to this injury.

Employee turned quickly when a rock fell from the roof. He heard a pop in his left knee. He continued to work until 4/13/06. At that time he had swelling in the knee and went to the doctor.
One employee was attempting to break up ice with an 8lb. Hammer while the injured emloyee was trying to remove it. from a belly pan. about the time the first employee swung the hammer, the victim started to remove a piece of ice. The hammer struck the middle finger on his right hand.

EE WAS ASSISTING WITH THE MOVING OF THE CONTINOUS MINER. HE PLACE HIS RIGHT HAND ON THE PAN AND BENT OVER TO PICK UP THE MINER CABLE. HE ASKED THE MINER OPERATOR TO RAISE THE PAN. WHEN IT WAS RAISED, HIS HAND WAS PINCHED BETWEEN THE PAN & THE STOP ON THE CUTTER HEAD. HIS HAND WAS BROKEN IN SEVERAL PLACES.

Ee was grinding a small piece of steel on a truck bumper. He was wearing a full face shield; apparently the protection was unable to shield him from a small piece of some foreign matter entering eye.
Employee stated that he was attempting to remove a rock from between the tailgate and the truck bed. When the rock dislodged, the tailgate swung back while he was holding onto pin and struck the latch.

EE was assisting in aligning used truck tires so they could be lifted into a dump trailer with a forklift. The ee stated that he pulled something in his back. EE returned to work on 7-19-04, worked 4 hours, went home.

EMPLOYEE WAS ATTEMPTING TO MOUNT THE 777D ROCK TRUCK. HE PLACED HIS LUNCH BOX ON THE BUMPER, THEN CLIMBED TO THE BUMPER. WHEN HE TRIED TO PLACE HIS LUNCH BOX ON THE CATWALK. HE DROPPED HIS BU CKET. WHILE ATTEPTING TO CATCH THE LUNCH BOX, HE SLIPPED AND FELL TO THE GROUND.

WHILE TRAMMING BRIDGES, THE OPERATOR DECK SLID UP ONTO THE SIDE OF THE LOOKON EE CLAIMS TO HAVE STRUCK HIS HEAD ONT HE CANOPY WHEN THE DECF SLID OFF THE CON LOW. HE CLAIMS TO HAVE INJURED HIS HEAD AND NECK.
