EE WAS STEPPING OFF SIDE OF SERVICE TRUCK, STEPPED ONTO PIECE OF COAL, FOOT SLIPPED AND BROKE ANKLE BONE.
EE BACKED UP TO THE DUMP AREA AND DID NOT STOP.
EE WAS ON HIS 8TH OR 9 TH POND WITH THE R-90 TRUCK. HE BACKED UP THE TRUCK AND DID NOT STOP AND THE TRUCK WENT DOWN THE DUMP AREA APPROX. 15 TO 20 FEET AND ROLLED OVER.
