Employee fell while dismounting Caterpillar D11 Bulldozer.

Employee was climbing up on a 360 Excavator to grease it and lost EE's balance, falling backwards hitting EE's back on the boom jack of the excavator

Employee was shoveling track on a dozer and stepped into a hole and fell. Employee stated EE did not see hole.

Employee was putting drill steel on drill up on mask. EE slipped and drill steel rolled over on EE's leg and pinched it on the mask of the drill.

Employee backed 777 rock truck into loader. Employee worked the rest of the ten hour shift, and did not tell anyone, on the job, that was hurting. Employee stated went to the doctor the next day 02/18 for neck and back pain. Employee called into office today 02/21 and reported the injuries.

Employee felt "stinging" in EE's shoulder while pulling self onto first step of excavator.

Was grinding weld down when a spec of material entered the eye. Employee was wearing safety glasses and face shield

Employee slipped while dismounting Mack Lube Truck after maintenance.

While descending the haul road, the brake pedal became spongy and the truck wouldn't slow down. The driver steered the truck into the ditch and then hit the escape tamp. The truck traveled up the escape ramp and then over the hill. The truck came to rest at the bottom of the hill on the lower road.

Employee slipped while dismounting Komatsu 475 dozer. Employee fell onto the track of the dozer on his right side.

Employee was opening a box of bogie pads when one fell out and landed on his left foot.

Employee was using cutting torch on a bucket when a hot peace of slag went into his ear. Employee first said he was all right, but started experiencing slight hearing loss. Employee has received medical treatment and has missed no work days.

EE was operating a 775 rock truck, hauling coal. He said as he approached the parking lot, the truck started to slide and gain momentum. He stated that it slid around and overturned onto the off-side of the truck.

Employee was carrying filters through filter trailer, slipped and fell on shoulder.

Employee was operating loader. Employee backed over auger hole and hurt his back.

Employee was attempting to start welder on back of service truck. Employee fell while attempting to step onto bumper. Employee caught himself with his right arm and hurt his right hand.

Employee was bitten by a copperhead while retrieving a hose from the hose trailer.

Employee was removing tooth from a 993K Loader bucket with hammer and metal bar. A small piece of metal stuck employee in the leg. He rolled up his leg and showed it to a supervisor. Supervisor said a tiny hole like a pen hole was on his leg. Employee finished working his shift.

Employee fell off of Komatsu 475 dozer. Employee hurt his back and arm.

Employee was operating Caterpillar 988 Loader. Employee was backing out of a pit of coal when the loader "jerked."

While servicing Komatsu 475 bulldozer, EE fell from top of dozer tracks, striking his back on the ground.

Operator reported back pain while operating bull dozer.

Operator exited excavator to clean tracks with shovel. Operator claims he felt something like a "bee sting" in his lower back while shoveling tracks on the excavator

Opening door on D11R dozer, the door was frozen, the handle broke and he fell to the ground. He hurt his elbow, left ribs and lower back.

EE was attempting to remove a drain plug from a 777 rock truck. He was trying to remove the drain plug by striking a wrench with a hammer. He missed the wrench and struck his hand with the hammer

Operator slipped on step of push arm on dozer, while mounting dozer. Left leg hit edge at push arm.

Employee stepped off of dozer and pushed the arm of the dozer. Got pain in lower back. Employee said he pulled muscle and drove himself from the job.

Employee was greasing equipment, when grease fitting exploded and struck him in left testicle.

Employee was dismounting 988 Loader and twisted his back.

Employee was trying to lift rock out of crusher machine. He pulled a muscle in his back.

Employee slipped and fell while greasing a D8 dozer. When he landed he hurt his ankle.

1993 Mach tractor/trailer (loaded) turned oer on elevated curve, causing small engine fire(app less than 5 mins). Driver left vehicle, assuming incident over. Apparently truck re-ignited after driver left causing another fire. No bodily injury occurred due to this accident.

Employee got on the passenger side of 785 rock truck to clean mirror. Said he tripped over CB coax cable, fell and hurt his back. Employee finished shift, drove himself home. Tried to work the next day but said his back was hurting too bad.

Operator was climbing into 775 truck. He slipped on the second step, about 2 feet from the ground, and landed on both feet. Operator claims he pulled something in his back

Operator got something into eye while operating a dozer on the valley fill area.

Operator exiting rock truck and closed door on his right thumb.

Employee backed rock truck through berm and over highwall. Employee jumped from truck, breaking his leg.

EE was operating D10R dozer, a rock slid from the side of blade of dozer and operator did not see it. Operator backed over rock, hurting his back.

Raining. Employee tarped his load & was in hurry to get in truck. His foot slipped on the running board & he fell.

Operator was exiting excavator and turned his ankle.

EE WAS CUTTING THE SIDE OF A LOADER FRAME APART & THE TORCH POPPED AND A PIECE OF HOT SLAG WENT DOWN INTO HIS EAR.

REPLACING RF SPRING ON GREASE TRUCK. HOISTING FRONT END OF GREASE TRUCK WHEN BOOM BENT DROPPING GREASE TRUCK AGAINST EE'S HEAD.

OPERATOR HIT THE TOE ROCK WITH 990 LOADER STOPPING LOADER SUDDENLY AND COMPLETELY.

EE BACKED TRUCK UNDER LOADER TO BE LOADED. FIRSTBUCKET CONTAINED LARGE ROCK IN DIRT & WAS DUMPEDBEFORE TRUCK WAS COMPLETELY STOPPED. BACK STRUCKSIDE OF BED THROWING DRIVER AGAINST DOOR & STEER ING WHEEL.
