Employee fell while dismounting Caterpillar D11 Bulldozer.

Employee was climbing up on a 360 Excavator to grease it and lost EE's balance, falling backwards hitting EE's back on the boom jack of the excavator

Employee was shoveling track on a dozer and stepped into a hole and fell. Employee stated EE did not see hole.

Employee was putting drill steel on drill up on mask. EE slipped and drill steel rolled over on EE's leg and pinched it on the mask of the drill.
