Employee was tempting to load a screen from a screening plant on the back of flat bed truck, it was not pushed up far enough and slipped off the end of the truck landing on their left foot. Employee was wearing steel toe boots and gloves. Left foot was bruised.

While helping load a large tank on a trailer to move it to another location, the injured cut EE's thumb on a sharp piece of metal requiring six stitches.

Injured backed 1 ton truck up on a 1 ft high bladed berm on level ground to replace a U-joint. EE put the truck in park, set the brake, and chocked the front wheels. With the drive shaft removed, only the front brake and chocks were left to hold the truck on the berm incline and they slid 2.5 foot forward on the muddy ground causing shoulder bruising by the rear dual wheels.

While helping remove branches stuck in a harrow, EE slipped causing EE to hit one of the spikes with arm causing a laceration that required stitches.

While operating a Broce Broom backing up, the injured hit a bump causing the seat to shift on the cushioned seat support pinching his finger. Injury is a broken finger tip from being mashed.

While reaching to turn off a valve on the highwall drill, his hand slipped exhaust pipe causing a burn.

While performing maintenance, the injured's wrench slipped causing him to hit the frame of the disc and lacerated his thumb requiring stitches.

While removing the grill cover on a Cat D11 Dozer, the hinge broke causing the grill to fall onto the injured's hand breaking one finger and lacerating this hand requiring stitches.

While changing out of his rain suit and rubber boots, the injured was putting on his normal work boots when he heard a loud pop and his foot started hurting. He fractured his foot while putting on his boots.

While being loaded, a large rock dumped into the bed of his end dump, rolled into the side of the bed causing the truck to twist from the blow which broke the window popping a glass shard to to fly and scratch his eye, requiring a presription for pain.

While removing a cylinder pin, the injured smashed his finger with a drive pin and hammer causing the need of stitches on the ring finger of his left hand.

While EE was using a hammer to pound a tooth keeper in on a Cat 992 Loader, a piece of metal chipped off and flew under his safety glasses and lodged in his eye requiring surgery to remove.

While driving a 10 wheel dump truck on a muddy road, injured slid sideways & struck sollid dry road causing the truck to turn over on it's side. The injured was wearing his seat belt, but hit the door with his shoulder & fractured it. ** The injured only works part time for the mine, two days a week.**

While welding on an end dump truck, sparks landed on the injured's clothes and caught them on fire. He suffered 1st, 2nd, & 3rd degree burns on his upper left torso from the wind blown fire.

While dismojnting from his bulldozer, the injury hit his elbow on a handrail and bruised it.

While moving a 6" water hose out of the way, it snapped back into the injured's body resulting in a fractured hand and strained knee.

While walking across the ground, the injured took a step and turned his body but his ankle did not turn with him resulting in a fractured ankle.

An unknown event caused a hernia injury during, but on an unknown date in late 2007. No lost time or medical until the injured had an operation to correct the hernia. Off work from 2/26/08 through 4/2/2008. The injury has been determined to be a work related injury and workers comp insurance is/has handled it.

Injured notified company on 10/29/07 of injury that happened sometime in July, 2007. While fueling a dozer, the injured lost his balance walking on the track and fell. He has been diagnosed with a herniated disc. Last date employee worked was 10/20/07.

EE was terminated for refusing to work. On his way off the minesite, he told the scale house lady, that allegedly, his back was hurt and he was going to the doctor. He had never before told the company of this alleged injury until after he was fired, and then only to the scale house clerk, not the boss that had just fired him.

While servicing equipment, a diesel fuel nozzle fell out of a fuel tank and hit the injured in the head over the left eye requiring stitches.

While operating his bulldozer, speck of dirt flew into his eye from the a/c vent requiring a doctor to remove the dirt from under his eye lid. No time lost or other medical treatment. returned to work on his next regular shift.

While operating an end dump truck, the alleged injured turned the truck over. He stated many times that he was not injured and refused to go to the emergency room/doctor to be checked out. He was tired for his activities causing him to turn the truck over. Two weeks later, an attorney contacted the company claiming numerous injures. (sore back, arm, etc)

While mounting his equipment with muddy boots, the injured slipped while sitting down in his seat & twisted his body causing a strain to his back, neck, hip and leg.

The injured stepped off the bumper of his mechanic service truck and his knee popped, resulting in a strain/sprain to his right knee.

No lost time until 5/1/06. No medical treatment unti 5/3/06. While backing up his bulldozer, the movement of the bulldozer jarred his back. He is receiving chiropractic treatment for the injury.

While dismounting a front end loader, the injured slipped and fell to the ground and broke his leg.

WHILE PERFORMING MAINTENANCE ON THE HIGHWALL DRILL, THE INJURED SLIPPED AND GRABBED THE EXHAUST PIPE TO CATCH HIMSELF AND PREVENT A FALL. BY GRABBING THE PIPE HE BURNED HIS ARM AND HAND.

No lost time or medical treatment until 11/8/05. Injured picked up a heavy tool and caused a bone chip in his back and hip area. The injured has an inherited degenerative disease that results in bone chips and injuries from lifting heavy objects. He must have surgery to remove the bone chip and rehabilitation before returning to work.

WHILE OPERATING HIS END DUMP TRUCK ON THE DUST CONTROL WATERED ROAD, THE INJURED SLID ON THE WET ROAD INTO THE BERM AND ROLLED THE TRUCK OVER. A SMALL LACERATION IN HIS HEAD REQUIRED 2 STITCHES TO CLOSE.

Employee slipped on Boom of P&H Shovel and slid into the hose/pivot area. causing bruises, cust and scrapes. Employee was transported to hospitol for observation and was released from hospital on 9-8-05

NO LOST TIME UNTIL 10/15/04. EE WAS CARRYING A 50 LB. PART INTO THE PARTS TRAILER WHEN HE FELT A TWINGE IN HIS BACK THAT RESULTED IN A STRAIN TO HIS BACK AND HIP.

WHILE LIFTING FIBER BOARD DRUMS TO EMPTY INTO A HOPPER, THE INJURED STRAINED HIS BACK.

WHILE WALKING ACROSS THE COAL PAD, THE INJURED SLIPPED ON ICE AND FELL TO THE GROUND, STRAINING HIS SHOULDER.

WHILE STEAM CLEANING A P & H 2250 SHOVEL, THE INJURED ACCIDENTLY SWEPT THE STEAM WAND ACROSS HIS FOOT AND BURNED IT.

WHILE DISMOUNTING FROM UNSTOPPING THE HOPPER OF A PORTABLE CRUSHER, THE INJURED SLIPPED AND CUT HIS ARM ON A SHARP PIECE OF METAL ON THE EQUIPMENT GUARD. THE CUT ON HIS ARM RESULTED IN 2 STITH ES.

WHILE HELPING INSTALL TIRE CHAINS ON A 992 LOADER, A CABLE THE INJURED WAS HOLDING SLIPPED LOOSE AND HE FELL STRAINING HIS SHOULDER AND NECK MISSED ONE DAY, WORKED TWO DAYS. AND THEN MISSED TW O ADDITIONAL DAYS FROM BEING SORE.

WHILE WALKING ON THE MINESITE, THE INJURED STEPPED ON A TIRE RUT AND STRAINED HIS ANKLE.

WHILE MOUNTING HIS SERVICE TRUCK, THE INJURED MISSED THE HANDLE AND FELL TO THE GROUND TWISTING HIS ANKLE. DOCTOR GAVE HIM A PRESCRIPTION AND SHOWED HIM TO RETURN TO WORK ON HIS NEXT SCHEDULED DAY.

THE INJURED HIT THE HEADLIGHT ON HIS WATER TRUCK TO TRY TO ADJUST IT. THE LIGHT BROKE AND CUT HIS WRIST REQUIRING STITCHES.

WHILE STEPPING OUT OF A TRUCK TO THE GROUND, THE INJURED STEPPED ONTO A RIDGE IN A MUD RUT THAT HAD DRIED AND TWISTED HIS ANKLE. THE ANKLE WAS WEAK FROM BREAKING IT AT ANOTHER JOB IN 1996 AND HE REFRACTURED IT.
