Employee was welding inside a belt head rock box. Subjected to reflective flash off metal plate behind him. Did not report incident until emergency room called mine for insurance details.

Cutting rubber hose with knife - hands were wet and mucky - slipped and cut across finger
A fall occurred in the #9 crosscut, along #1 belt line. The roof fell above the anchor of the 4ft.resin bolts and comprised 5ft of gray, slickensided shale. It measured 40ft x 25ft and it crushed 4 sections of conveyor structure. The area was posted and cribbed, and intersections from #6 to #9 break were bolted with 10ft cable bolts.
There was an unintentional fall of roof in the #8 crosscut, off #3 entry at the Clark No.1 mine. Debris from the fall knocked 2-3 blocks out of the return side stopping, this however did not interrupt section ventilation as we had two additional return airways. The area was cribbed off to prevent further roof issues.
An unintentional fall of roof occurred at Spad No.1032, in the #3 entry, at the Clark No.1 Mine. The roof fall blocked the flow of air from the right split off section 001. The roof at the location of the fall comprised mostly slickensided shale. Area cribbed off and ventilation was restored. MSHA informed at 4:10 am.
Employee was getting water when right foot was placed on ground. His right ankle rolled and twisted, causing swelling and pain.
Roof Fall in Room #3 intersection S pad 2 and 30. Area was timbered off and cribs built.
Walking down belt line and slipped resulting in hyper extended knee
Roof Fall - occurred at intersection of cross cut #15 in the belt line entry. Involved in the fall was #1 Belt line (structure, water line and air line) It was cover with approx. 8-9 ft of gray shale rock. Cause of accident was geological conditions.
Water line hit employee on left side of face while moving 2" air pump
Employee walking around miner while the miner was trimming from #4 to #6 and was crushed between the tail boom and the rig. His injuries were mid-section and left leg bruising.
Working on power head and twisted the wrong way while removing cover plate.
Twisted knee while walking.
Employee was working outdoors in high temperature. Heat index high 90 degrees - low 100 degrees. Suffered heat prostration vomiting etc. Very dehydrated.
Employee was pulling a water hose when the hose gave way. Employee hit his right elbow against a handrail.
Taking mantrip to section, stated that rock fell from roof and hit him in the chest. Carried onto section, got off the mantrip and collapsed.
EE was loading tram motor for transport. EE asked for a sling to be thrown to him, the sling was thrown to him but one end caught a control lever on the Bobcat. The EE pulled on the sling which activated the control lever causing the Bobcat to run up on his leg.
Employee was coming down steps and accidently bumped into beam.
Employee trying to pull slack on CB radio coaxial cable & cut a zip tie with pocket knife when knife slipped off and cut his left index finger. Had 7 stitches in wound at E.R. and returned to work at 11 PM.
The EE was standing next to No.3 conveyor tail section observing the tracking of the belt when a stiff jack dislodged and struck him on the upper body. He received contusions to his right side chest wall.
EE was removing hydraulic pump from Mack Truck, the pump was suspended by a rope which broke allowing pump to fall on EE's fingers (2) on right hand.
Employee's were taking top of screen bowl. The chain attached to the cover slipped letting the cover hit the employee on top of his foot.
Injured was trying to unload a 15 hp pump from a scoop bucket.
Employee was working inside a screen with limited space to work. He pulled hard with a long handle rachet, he felt something pull in his left shoulder.
Injured was walking behind scoop and walked into a roof strap.
Injured fell on his leg while moving a shuttle car cable. He had swelling of his ankle and went to emergency room. He was diagnosed with pulled ligament and did not miss work.
EE was in a manlift installing a camera line. He tripped over his lanyard and grabbed handrail w/his left hand and started to grab it w/his right hand also (which he was holding a box cutter in) and cut his left index finger, which required some stitches.
Chute feeding the scalping screen had plugged causing a few rocks to fall on floor below. The individual had cleared the chute and was going back to bottom floor of structure. The laborer did not have time to get to spill at this time. The individual stepped on one of the rocks causing him to fall onto the floor grating.
THE INJURED STUCK A PRY BAR INTO THE VALVE OF A SUMP, TRYING TO BREAK UP THE MATERIAL. HIS HAND BECAME WEDGED BETWEEN THE VALVE AND THE PRY BAR RESULTING IN INJURY.
WAS PULLING BOLTER CABLE, SLIPPED IN GOB AND TWISTED ANKLE, THIS SAME ANKLE WAS SPRAINED BEFORE,BUT WAS NOT REPORTABLE AT THE TIME.
WAS PULLING H V CABLE - SLIPPED & FELL AGAINST MINE FLOOR & PULLED BACK MUSCLE.
STRAINED HIS BACK WHEN MBC BROKE LOOSE - WAS NOT REPORTABLE AT THE TIME - TOLD TO GO ON LIGHT DUTY 8/21/02 - LOST NO TIME. 10/3/02 HAD MORE PAIN - NOW REQUIRING FOLLOW UP TREATMENT.
PARTY WALKING DOWN SLIPPERY SLOPE, FELL AND HIT ELBOW AGAINST FLOOR WITH WEIGHT OF BODY BEHIND IMPACT.
VANDALS BROKE INTO LOAD-OUT, RAN COAL THROUGH BELT SYSTEM, DROPPED LOAD-OUT CHUTES TO SPILL COAL ON RAIL TRACKS. CONTROL ROOM WAS SET ON FIRE. SHERRIFF DEPT. & FIRE MARSHAL INVESTIGATED SAME. EXTENSIVE DAMAGE DONE. FACILITY USELESS NOW.
MBC UNIT WERE BACKING UP INCLING IN BELT ENTRY WHEN A CONNECTING HINGE PIN BROKED THIS SUDDEN MOVEMENT CAUSED THE OPERATOR TO BE THROWN OUT OF HIS SEAT AND AGAINST A BIG ROCK.
EMPLOYEE WAS DRILLING THE MINE (AND WAS STRUCK BY A PIECE OF SHALE 18" X 18" X 3" RESULTING IN A BRUISE).
EE STEPPED INTO A HOLE WHILE COUPLING WATER HOSE & PUT HIMSELF OUT OF POSITION FOR THE TASK AT HAND - CAUSING HIM TO TWIST HIS BACK.
EE RIGHT FOOT TURNED SIDEWAY WHILE CLIMBING DOWN FROM END LOADER.
EMPLOYEE WAS HANGING LINE CURTAIN FROM ROOF TO CATCH A DRIP FROM TOP. WHILE HE WAS IN THE PROCESS OF HANGING CURTAIN HE SLIPPED AND FELL ON HAMMER CAUSING A CUT ABOVE RIGHT EYE.
EE WAS REMOVING COVERING OFF MOTOR-SLIPPED AND FELL WITH FACE COMING INTO CONTACT WITH OIL TANK.
WHILE LOADING SCOOP BEHIND BRIDGE CONVEYOR, EE WAS OPERATING STEERING, IN BACK AND FORTH MOTION TO LOAD BUCKET EVENLY, FROM OUTSIDE OF SCOOP DECK. EE'S LEG GOT CAUGHT IN PINCH POINT AREA OF SC OOP AND INJURED HIS LEFT KNEE.
WHILE INSTALLING ROOF BOLT WITH HEADER BOARD, BOARD FELL APPROX. 3' CATCHING EE'S HAND BETWEENBOARD & BOLT WRENCH.
WHILE EE WAS GETTING OUT TO SCOOP DECK, HE SLIPPED AND FELL STRIKING KNEE AGAINST GROUND CAUSINGINJURY.
HANGER BROKE WHILE HE WAS HANGING VENTILATION TUBING AND TUBING FELL ON CLAY, PUSHING HIM TO THEGROUND, BENDING HIM DOUBLE AND CAUSING HIS SCSR TO PUSH INTO HIS RIB & CHEST AREA CAUSING INJURY
LOSE ROCK FELL FROM TOP HIT EE IN BACK.
