While attempting to place a battery charger in a rack, the charger slid suddenly and mashed fingers in the rack. Mashed ring and middle fingers of right hand.

A roof fall occurred on the No. 1 section in the #3 entry cross cut at spad 7308. The roof fall measured approximately 25 feet long by approximately 22 feet wide, and approximately 15 feet in height.

Employee was installing a roof bolt when the employee got his right index finger pinched between the roof bolt plate and the mine roof. Employee did not become a lost time accident until he had surgery on his right index finger on 10-29-08.

Employee was walking past the #4 belt header when employee struck his head against a roof bolt head and plate on a installed cable bolt.

The continuous miner was cutting in the #4 right face, as the miner was cutting on the left side of the face an ignition occurred in the left corner.

The miner was cutting the top in the #1 face on the #1 section when a methane ignition occurred in the mine roof.

Employee was helping the roof bolter operator install a 6 foot resin bolt, as the employee was putting the head of the bolt in the bolt wrench, a rock fell and hit the emloyee on the head and shoulder area.

Employee was helping re-direct the fire suppression sprsy outlets at #2 belt header, when employee slipped and landed on his rescuer on his right side. The area at the header is on a steep grade and wet.

A roof fall occurred on the No. 1 section in the number 3 entry cross cut at spad 7135. The roof fall measured approximately 30 feet long, 30 feet wide, and approximately 9 feet in height. The roof fall fell on the No.2 MBC, No. 3 MBC,and portion of the belt tailpiece. The No.2 MBC operator was trapped inside the operator compartment for a brief time span but suffered no injuries.

Employee was getting tools out of tool box, when tool box lid fell, hitting employee on the left hand.

A roof fall occurred approximately 2 feet inby the arch installation at the slope entrance in the #3 entry. The roof fall measured approximately 7 feet high by approximately 15 feet wide by approximately 20 feet long.

The employee was helping make a belt splice on the #1 belt, when the belt splicing tool got jammed. As the employee was helping to free up the belt splicing tool, his left hand got caught, causing a fracture to his left thumb.

A methane gas ignition occurred on the #1 section at approximately 7:30 am. The continuous miner had cut approximately 20 feet out of the face area in the #3 left cross cut and was cutting in the top when the methane ignition occurred.

Employee slipped and fell on the No.1 Section, causing a fracture to the bone between the left thumb and the wrist.

A methane ignition occurred on the #1 section, in the #4 face, while the miner was cutting on the left side of the face.

Employee was helping remove a bad pump motor on the miner, the chain hoist that was being used to remove the bad pump motor broke loose and hit the employee on the left knee.

Employee was operating a Lo-Trac (Light Duty Forklift) on the #1 section. As the employee was tramming the Lo-Trac, his right arm was caught between the mine roof and the frame of the equipment.

A methane gas ignition occurred on the #1 section. The miner was starting a new cut in the # 3 right face, As the miner was cleaning up the coal accumulation on the exhaust curtain side a ignition occured in the bottom area.

A roof fall was discovered at approximately 1:35 pm on 10-29-07 on the abandon old #1 right section in the #2 entry inby spad 6505. The height and distance of the roof fall cannot be determined due to geology condition.

A methane gas ignition occurred during mining in the No. 2 entry on the No. 1 section. The miner had cut approximately 20 to 25 feet out of the face, as the miner was trimming the roof on the exhaust curtain side the ignition occured.

Employee was walking from the power center to the miner on the No. 1 section when the employee twisted his left knee.

Employee was making a weekly examination in the return, as employee was walking up the slope, he slipped and twisted his left foot and knee.(Employee had surgery on his left knee and started missing work on 9-12-07 because of the accident he had on 8-29-07.)

Employee's right hand was caught between a set timer and the shuttle car frame as he was traming the shuttle car to the miner with his right hand located on the outside frame of the operator compartment. (Fracture and laceration requiring 5 sutures to right hand little finger.)

A roof fall was discovered at approximately 2:10 am on the 1-right section inby spad 6499 in the #3 entry by foreman. The area around the roof fall has been dangered off. The height and distance of the roof fall can not be determined due to geological conditions and accumulation of water in the face area.

A roof fall was discorved by EE during a weekly examination on owl shift August 15, 2007. The roof fall is located on the old 2- left section in the intersection of #3 entry at spad 6274. The fall measured approximately 20 feet wide, 30 feet long and approximately 10 feet high.

A roof fall occurred in the #4 entry on #1 right section at approximately 5:10 am on Monday August 13, 2007. The roof fall is located between spad 6329 and 6351. The roof fall measured approximately 60' long, 20.5' wide and 9' high.

A roof fall occurred on the #3 main line belt at approximately 1:00 pm on Friday August 10, 2007. The roof fall is located approximately 37 feet inby spad 4063 and approximately 38 feet outby spad 4234. The roof fall measured approximately 200' long, 21 feet wide and 9' high.

A roof fall occured approximately 4 feet inby spad 6517 in the intersection of the #2 face on 1-right section. The fall measured approximately 18' 6"" wide, 33' long by 9' high.

Employee lost control of personnel carrier as he was traveling down the slope into the mine. The employee went to set the emergency parking break when employee cut his right lower arm causing laceration to lower arm that required 4 sutures.

Employee was using a razor knife to open a rock dust bag when employee cut his left finger. The laceration required 3 sutures.

The roof in the crooscut between spads 6268 and 6258 fell above the anchorage. No person or equipment were involved. Mining was recently stopped in the area and no work activity was taking place at the time of the fall.

A remote discharge boom located at the #2 belt drive broke free from the roof pulling back and to the side striking the victim on the left side of face and head.

A spark from the miner bits ignited a bleeder of methane gas near the footwall. The flame was described as a flash that was put out with rock dust.

Horses back in area bolted with 42"" bolts

Blocking up tailpiece when rapid haul was pulled up catching finger between rapid haul tailpiece and crib block.

Caught foot between rib and mantrip resulting in broken foot.

While removing rock from a tail piece with a 2 X 4 the rock caught the 2 x 4 mashing his finger against the roof.

WAS PULLING BOLTER CABLE, SLIPPED IN GOB AND TWISTED ANKLE, THIS SAME ANKLE WAS SPRAINED BEFORE,BUT WAS NOT REPORTABLE AT THE TIME.

WAS PULLING H V CABLE - SLIPPED & FELL AGAINST MINE FLOOR & PULLED BACK MUSCLE.

STRAINED HIS BACK WHEN MBC BROKE LOOSE - WAS NOT REPORTABLE AT THE TIME - TOLD TO GO ON LIGHT DUTY 8/21/02 - LOST NO TIME. 10/3/02 HAD MORE PAIN - NOW REQUIRING FOLLOW UP TREATMENT.

PARTY WALKING DOWN SLIPPERY SLOPE, FELL AND HIT ELBOW AGAINST FLOOR WITH WEIGHT OF BODY BEHIND IMPACT.

MBC UNIT WERE BACKING UP INCLING IN BELT ENTRY WHEN A CONNECTING HINGE PIN BROKED THIS SUDDEN MOVEMENT CAUSED THE OPERATOR TO BE THROWN OUT OF HIS SEAT AND AGAINST A BIG ROCK.

EMPLOYEE WAS DRILLING THE MINE (AND WAS STRUCK BY A PIECE OF SHALE 18"" X 18"" X 3"" RESULTING IN A BRUISE).

EE STEPPED INTO A HOLE WHILE COUPLING WATER HOSE & PUT HIMSELF OUT OF POSITION FOR THE TASK AT HAND - CAUSING HIM TO TWIST HIS BACK.

EE RIGHT FOOT TURNED SIDEWAY WHILE CLIMBING DOWN FROM END LOADER.

EMPLOYEE WAS HANGING LINE CURTAIN FROM ROOF TO CATCH A DRIP FROM TOP. WHILE HE WAS IN THE PROCESS OF HANGING CURTAIN HE SLIPPED AND FELL ON HAMMER CAUSING A CUT ABOVE RIGHT EYE.

EE WAS REMOVING COVERING OFF MOTOR-SLIPPED AND FELL WITH FACE COMING INTO CONTACT WITH OIL TANK.

WHILE LOADING SCOOP BEHIND BRIDGE CONVEYOR, EE WAS OPERATING STEERING, IN BACK AND FORTH MOTION TO LOAD BUCKET EVENLY, FROM OUTSIDE OF SCOOP DECK. EE'S LEG GOT CAUGHT IN PINCH POINT AREA OF SC OOP AND INJURED HIS LEFT KNEE.

WHILE INSTALLING ROOF BOLT WITH HEADER BOARD, BOARD FELL APPROX. 3' CATCHING EE'S HAND BETWEENBOARD & BOLT WRENCH.

WHILE EE WAS GETTING OUT TO SCOOP DECK, HE SLIPPED AND FELL STRIKING KNEE AGAINST GROUND CAUSINGINJURY.

HANGER BROKE WHILE HE WAS HANGING VENTILATION TUBING AND TUBING FELL ON CLAY, PUSHING HIM TO THEGROUND, BENDING HIM DOUBLE AND CAUSING HIS SCSR TO PUSH INTO HIS RIB & CHEST AREA CAUSING INJURY
