TOTAL ELECTRICAL OUTAGE FROM UTILITIES SUB STATION DUE TO ELECTRICAL STORM, CAUSED HOISTS #1, #2, #4, #5 & ALL UNDERGROUND & SURFACE TO BE WITHOUT POWER. POWER RESTORED AT 10:00PM.
INTERUPTION OF POWER TO #1 & #4 HOISTS FROM THE UTILITY COMPANIES SUB STATION. FAULT OF ELECTRICAL OUTAGE IS ON THE UTILITIES COMPANY AT THE SUBSTATION. HOISTS BACK UP AT 2:00PM
#4 HOIST DOWN DUE TO SHEARED KEYWAY IN SHAFT FOR ROTOTROL. SHAFT WAS REMOVED AND NEW KEYWAY WAS CUT. #4 HOIST WAS RETURNED TO SERVICE AT 10:00 AM ON 8/4/04. UNDERGROUND MINERS HAD TWO OTHER SHAFTS FOR ESCAPEWAY OUT OF MINE.
#5 hoist down from 1:45 to 4:05 on this date. This was an electrical problem due to lightening. All other hoists operable.
EMPLOYEE WAS STANDING ON SECOND STEP OF A FOUR STEP LADDER TIGHTENING THE BOLTS ON THE FEED END OF THE #1 ROD MILL WHEN HE SLIPPED OFF OF THE LADDER AND FELL TO THE FLOOR. HE CAUGHT HIMSELF WHEN HE HIT THE FLOOR WITH HIS LEFT HAND INJURING HIS LEFT WRIST. ICE, WRAPPED AND IBUPROPHIN. EMPLOYEE LATER COMPLAINED OF INJURY STILL HURTING. WAS TREATED ON 9/1/04.
EMPLOYEE WAS STANDING ON LADDER USING HAMMER TO REMOVE RUST FROM OLD BEAM SO HE COULD LAY OUT HIS HOLE PATTERN ON OLD BEAM. WHILE HAMMERING, AN OBJECT FROM ABOVE FELL AND HIT THE EMPLOYEE IN THE LEFT ARM.
CONTRACTOR WAS A PASSENGER ON 340 MANTRIP. WHEN APPROACHING AN AIR DOOR THE VEHICLE WAS STOPPED TO PULL THE ROPE THAT OPERATED THE SET OF ELECTRICAL CONTACTS TO THE AUTOMATIC AIR-OPERATED VENTILATION DOOR. AS VEHICLE STARTED THROUGH THE DOOR ABRUPTLY BEGAN TO CLOSE. IT STRUCK THE VEHICLE AND SLIPPED OVER INTO THE PASSENGER COMPARTMENT AND STRUCK THE CONTRACTOR.
EMPLOYEE WAS CUTTING AN OLD TOOL BOX FROM BACK OF HIS VEHICLE WHEN THE LID CAME DOWN AND SEVERED HIS RT.HAND, LITTLE FINGER FROM THE FIRST JOINT.
Employee was jackhammering a concrete footing. The footing had been excavated. A piece of concrete the employee was hammering broke off causing the hammer to drop down into the excavation. The employee stretched, partially tore, his trepezius muscle in his right shoulder.
#2 HOIST DOWN DUE TO POWER OUTAGE @ 1:30 AM. POWER RESTORED AT NOON. #5 HOIST DOWN @ 5:00 AM DUE TO ELECTRICAL PROBLEMS. #1 & #7 NOT RUNNING AND MAINTENANCE #4 SHAFT THE ONLY ONE AVAILABLE.

CO-WORKER HEARD EE SCREAMING FOR HELP & FOUND EE BY THE BOOM OF THE CONTINUOUS MINER WITH HIS BACK TO THE RIB & THE BOOM IN HIS CHEST. CO-WORKER MOVED BOOM AWAY FROM EE'S CHEST.
HOOKED ALL AIR TO CARS & STARTED DOWN HILL WITH SHUTTLE WAGON UNDER CONTROL. LOST AIR PRESSURE & CONTROL OF RAILCARS. EE JUMPED OFF SHUTTLE WAGON AND LANDED ON BOTH FEET.
EE WAS PULLING A 3/4" WIRE ROPE APPROX. 300' LONG W/312 MANTRIP. THE WIRE ROPE WAS ATTACHED TO THE MANTRIP AT THE CENTER OF REAR BUMPER A CROSSBY CLAMP ON THE OTHER END OF THE ROPE CAGUTH BETW EEN THE VENT DOOR AND THE HINGE POST. THE VEHICLE STOPPED ABRUPTLY THROWING EE INTO STEERING WHEEL. EE S FORWARD BODY MOTION CARRIED HIM PAST THE STEERING WHEEL ON THE GROUND.
EMPLOYEE WAS INSTALLING LIGHT BRACKET AND ADJUSTING BOLTS. THE SWITCH BOLTS SLIPPED AND CAUGHT LEFT HAND BETWEEN LIGHT SWITCH AND L GROOVE.
EE WAS CAD WELDING GROUND BED. WHEN LIGHTING WELD CHARGE BLEW OUT AT TOP OF MOLD. PART OF THE CHARGE SHOT INTO LEFT GLOVE RESULTING IN BURNS. WELD WAS SET WITH RIGHT HAND. ALL PPE WAS IN PLACE I.E. LEATHER GLOVES, FARE SHIELD, LONG SLEEVE SHIRT.
AS EE WAS MOUNTING 7917 FRONT END LOADER HE MISSED THE FIRST STEP ON THE LADDER. THE EE HAD A HOLD ON RIGHT SIDE OF LADDER HANDLE AND WHEN HE MISSED THE STEP HIS RIGHT ELBOW STRUCK THE LOADER FINDER HANDLE CAUSING HIS ELBOW TO BEND BACKWARDS.
#2 HOIST WENT DOWN DUE TO SPEED TRACKING AT 10 PM. CLEARED ITSELF, UP AT 10:47 PM. LOCAL MSHA OFFICE NOTIFIED.
WHILE RETURNING TO CONTROLLER TO PICK UP TOOLS, EMPLOYEE MADE A MISSTEP ON AN 8" TO 11" SHELF IN FRONT OF THE BELT CONTROLLER. RESULTED IN INJURY TO ACHILLES TENDON ON LEFT FOOT.
MINER HAD BEEN MOVED ACROSS PANEL ON NIGHT SHIFT, BUT CABLE HADN'T BEEN HUNG UP. 2 EE'S WERE HANGING MINER CABLE ACROSS THE ENTRIES, THE BACK IN THAT ROOM MEASURED 78"-84". EE STATED THAT HE F ELT BURNING SENSATION IN HIS GROIN AREA.
A PIECE OF 1/4" MILD STEEL PLATE 32"X 60" WAS BEING LOADED ONTO A WELDING VEHICLE. THE STEEL WEIGHED APPROX. 136 LBS. THE PLATE WAS LIFTED UP & LEANED AGAINST THE VEHICLE. EE PICKED UP THE EDG E THAT WAS AGAINST THE FLOOR TO SLIDE IT ONTO THE VEHICLE & HE FELT A SMALL TWINGE IN HIS BACK.
EE WAS UNLOADING BELT STRUCTURE FROM BELT SKID. WHILE RAISING UP; PICKING UP A STAND HE TURNED TO THE LEFT TO PLACE STAND NEXT TO BELT & HIS LEFT KNEE POPPED & GAVE OUT. HE STOOD UP & FLEXED H IS LEG BACK & FORTH. WHEN HE CLIMBED OVER THE BELT HE FELT A SHARP PAIN. THE KNEE BEGAN SWELLING & PAIN WAS FELT WHEN HE WOULD PUT WEIGHT ON IT. HIS KNEE WAS ICED & HE WAS TAKEN TO ER.
#4 HOIST WENT DOWN DUE TO ELECTRICAL PROBLEMS (MG SET ARCHING). WILL REMAIN DOWN OVER THE WEEKEND UNLESS IT IS NEEDED, THEN IT WILL BE TURNED ON.

INDEPENDENT OWNER/OPERATOR TRUCK DRIVER ARRIVED AT THE IMC SURFACE OPS. PARTS WAREHOUSE WITH A DELIVERY OF CONVEYOR BELTS. A CONTRACT FORKLIFT OPERATOR BEGAN TO OFF LOAD (2) 500' ROLLS OF 30" CONVEYOR BELTING, WEIGHING 3100 LBS. AND THE TRUCK DRIVER WALKED AROUND THE BACK OF THE TRAILER, OUT OF SIGHT. THE END OF THE FORKS TIPPED CAUSING THE BELTING TO ROLL OFF STRIKING THE DRIVER,
WEAR OF THE GUIDE SHOES CAUSED SHOES TO DIG INTO THE WOODEN GUIDES, CAUSING ELECTRIC COIL TO FAIL IN THE AUTOMATIC MODE. TWO OTHER MAN SHAFTS WERE AVAILABLE FOR EVACUATION OF MINERS.
EE WAS DRIVING 10% SLOPE AND APPARENTLY LOST CONTROL OF THE VEHICLE AT THE BOTTOM OF SLOPE. THE VEHICLE DROVE THROUGH A PLYWOOD BULKHEAD AND CAME TO REST WHEN IT STRUCK A RIB.
AFTER MINING THE CENTER PASS IN R-107 EE MOVED THE MINER OVER & STARTED MINING THE R PASS. AFTERLOADING A COUPLE OF CARS IN THE PROCESS OF LOADING ANOTHER CAR THIS RIB BLEW OUT, HITTING EE & PINNING HIS LEGS UNDER A SLAB, APPROX 8"TO 10" THICK FROM THE CALF OF HIS LEG DOWN. THE ENTIRE SLAB WAS 14'X22'X80'. EE WAS STANDING BY THE BACK SHUTTLE CAR TIRE AND WAS BLOWN INTO THE TIRE
EE WAS HELPING TO INSTALL NEW ROPE AN #1 HOIST. EE USING A 2X12 BOARD UNCONTACT WITH SPOOL TO ACT AS A BRAKE. NEW ROPE WAS ATTACHED TO OLD ROPE USING 7/8" CHOKER WITH EYES CUT OFF AND TWO 1 3/ 4" CABLE CLAMPS ON EACH ROPE END. HOIST USED TO PULL NREW ROPE OVER CROWN SHEAVE & INTO HOIST HOUSE. THE CONNECTION GOT CAUGHT ON I BEAM, SOME 12-14' BELOW CROWN SHEAVE, ROPE FELL OFF SHEAVE.
WHILE DRAGGING THE ROADWAY IN AREA 21 OFFSIDE TRAILER TIRE OF685 RAMCAR HIT CORNER OF RIB R73 BI36 CAUSING A ROCK TO STRIKE EE ABOVE LEFT EYE CAUSING A LACERATION APPROX 31/2" LENGTH. THE ROCK HIT THE TOP FRAME OF GLASSES.
ELECTRICAL PROBLEMS @ #4 HOIST.
#4 HOIST DOWN. LIGHTENING STRUCK NEAR SPS #1 OR THE SUB C #3 SHAFT. THE BREAKERS FOR FAN @ #3 WERE RESET. THE UNDERGROUND FEEDER C #4 AS WELL AS #4 HOIST.
HOIST OUTAGE BECAUSE HOIST WOULDN'T RESET ON POWER. CORRECTIVE ACTION: CHARGED OUT A VOLTAGE FEEDBACK CIRCUIT CARD.
CONDUCTED BRAKE TEST TO IF HOIST COULD OVERRIDE BRAKES. POWER ELECTRICAL SURGE CAUSED HOIST TO NOT POWER BACK UP (SELF-CORRECTED). ELECTRICAL SURGE CAUSING PREMATURE ELECTRICAL PROBLEMS.
EMPLOYEE BENT OVER TO PICK UP FULL WATER BOTTLE AND AS HE LIFTED HE FELT PAIN IN HIS LOWER BACK.
NO POWER TO #5 4160 BREAKER SHOWED OUT. MSHA CARLSBAD OFFICE FIELD OFFICE WAS NOTIFIED BY PHONE OF THE OUTAGE.
LILLY CONTROL CONTROLLER WAS STICKING, REPLACED RIGHT LILLY CONTROL CONTROLLER. MSHA CARLSBAD FIELD OFFICE WAS NOTIFIED BY PHONE OF THE OUTAGE.
EE HAD STARTED REMOVING THE BOLTS ON 4" FLANGE TO THE PILOT AIRLINE TO THE KMAG DRIVE BURNER. THE EE ATTEMPTED TO KNEEL NEXT TO THE FLANGE & STEPPED ON HIS OWN 15/16" COMBINATION WRENCH. TH IS CAUSED HIM TO SLIP FORWARD & STRAIN HIS OUT RIGHT KNEE
EE WAS PULLING 1 1/2" RUBBER HOSE UNDER THE D-40THICKENE TO THE 50'U/F WASHOUT. EE WAS PULLING THE HOSE AT HIS SIDE WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.(RX FOR PHYS.THERAPY DAILY FOR ONE WEEK).
MOISTURE IN THE AIR DUE TO RAIN, CAUSED A PHASE TO GROUND FAULT IN THE #2 HOIST POWER, INSLATORSWERE REPLACED, MINERS WERE TRANSPORTED DOWN THE #4 SHAFT. MSHA CARLSBAD FILED OFFICE WAS NOTIFIED BY TELEPHONE OF THE OUTAGE.
