CHANGING INTO HIS WORK CLOTHES, HE REMOVED HIS DRESS SHOES. HE GOT UP TO WALK OVER TO GET SOMETHING. HE STRUCK HIS FOOT AGAINST BENCH POST.
SERVICE HOIST SLOW SPEED RELAY DEVELOPED FAULT PUTTING HOIST OUT OF SERVICE IN EXCESS OF ONE HOUR. MSHA WAS NOTIFIED AFTER 30 MIN AND MINE PERSONNEL WERE EVACUATED AT 5:40PM. MINE PERSONNEL SE NT BACK AFTER REPAIRS WERE COMPLETE AT 6:05PM.
MACHINE BROKE IN HALF AT PIVOT POINT.
THE SURFACE ELECTRICIAN WAS GOING TO CONDUCT HIS ROUTIVE TEST RUN OF THE SERVICE HOIST WHEN HE FOUND IT TO BE INOPERABLE AFTER 30 MIN. MSHA WAS NOTIFYED AND MINE WAS EVACVATED. DAYSHIFT EE DID NOT ENTER THE MINE. MIDNIGHT SHIFT EE WERE EVACVATED WITH IN ONE HOUR BY THE PRDOCUTION HOIST. SERVICE HOIST BACK IN
MECHANIC WORKING ON TIP OF BOOM OF 988 CAT LOADER. WHEN CLIMBING DOWN HE SLIPPED AND CAUGHT HIMSELF WITH HIS RT ARM. SUDDEN JERK AND TWIST CAUSED A MUSCLE TEAR AND PARTIAL SEPARATION OF RT SHO ULDER. EMP HAS SINCE BEEN OUT OF WORK.
EE WAS STRIKING LATCH PIN WITH A HAMMER, ON AIR LOCK DOOR, WHICH HAD GOTTEN BENT, IN AN ATTEMPT TO OPEN DOOR. BOLT BROKE OFF, STRIKING AND CUTTING EE ON CHIN.
CUTTING PLASTIC BAG OPEN WITH UTILITY KNIFE AND KNIFE SLIPPED CUTTING EMPLOYEE'S INSIDE RIGHT THIGH, REQUIRING SUTURES.
WHILE HAND PALLETIZING 50 LBS BAGS, ONTO PALLET , EE FELT A MUSCLE PULL IN HIS LOWER BACK. TO WORK ON RESTRICTED DUTY.
EE STEPPED ON LOOSE STONE, SLIPPED, FELT PAIN IN BACK. PRESENTLY SEEING A CHIROPRACTER.

2 MECHANICS WERE ATTACHING A PLOW ATTACHMENT TO A MECHANICAL SCALER. THE INJURED EMPLOYEE WAS HOLDING A CONNECTING LINK IN PLACE WHILE A CO-WORKER WAS INSTRUCTED TO DRIVE A PIN THRU IT. THE HA MMER GLANCE OFF THE TIRE OF THE MACHINE AND STRUCK THE EMPLOYEE IN THE LEFT HAND AND INDEX FINGER.
EMPLOYEE WAS GETTING OUT OF TRUCK,SLIPPED ON SLICK SALT, HITTING HIS JAW ON VEHICLE, KNOCKING OUT THREE TEETH. EMP DID NOT SEEK MEDICAL ATTENTION UNTIL 9-21-95.
EE WAS ABOUT TO DRIVE UNDER AN OVERHEAD DOOR WHEN THE PULL CORD CAUGHT ON HIS VEHICLE, CAUSING DOOR TO COME DOWN. EE HIT DOOR WITH HIS HEAD. FOUR SUTURES OVER HIS LEFT EYE.
THE PRODUCTION HOIST WAS INVOLVED IN AN ACCIDENT ON JULY 6. THE INITAL INVESTIGATION REVEALED THAT A LOADED WEST SKIP HAD SLIPPED THRU THE BRAKES AND CRASHED INTO THE DIVIDING TIMBERS AT THE S HAFT BOTTOM. THE EAST SKIP STOPPED AT THE CRASH BEAMS IN THE HEADFRAME. THE HANDRAIL AROUND THE EAST SKIP WAS CRUSHED AND INSPECTION OF THE ROPES SHOWS NO IMPACT OR DAMAGE. THE WEST SKIP SUFF
WHILE INSERTING ROOF BOLT INTO HOLE. THE EMPLOYEE PLACED THE DRILL STEEL NEXT TO THE TRANSMISSION OF T EBOOM. WHEN HE RAISED THE BOOM THE STEEL STRUCK THE TRANSMISSION AND STRUCK EMPLOYEE IN THE RIGHT ARM.
LIFTING SIDE RAILS ON POWDER TRUCK. SIDE PANEL GOT STUCK. EMPLOYEE ATTEMPTED TO FORCE RAIL OUT OF SLOT AND STRAINED WRIST.
WHEN BOLTING TROUGHING IDLERS TO CONVEYOR, CO-WORKER RESET EMERGENCY STOP CARD. START WARNING HORN WAS IGNORED & EMPLOYEES ARM WAS DRAWN THROUGH IDLER. BOTH MECHANICS FAILED TO FOLLOW COMPANY LOCKOUT PROCEDURES. BOTH MECHANICS HAD LOCKS ON THEIR MINE BELTS.
EMPLOYEE WAS DOING REPAIR WORK ON AN UNDERCUTTER RAISING A TRACK-JACK. THE JACK HANDLE (PRY BAR) BROKE, SPRINGING UP AND STRIKING EMPLOYEE IN THE CHEST.
EE WAS DRILLING A HOLE INTO THE SALT ROOF FOR A SURVEY POINT, WHEN SALT FELL INTO HIS EYE. HE WAS WEARING SAFETY GLASSES. EE WAS GIVEN PRESCRIPTION FOR EYE DROPS.
AIR SHAFT HOIST EXPERIENCED ELECTRICAL PROBLEMS THAT AFFECTED COMPUTER CONTROLS. RE-PROGRAMED COMPUTER.
WHILE BACKING UP A PICKUP TRUCK, EE TURNED HIS HEAD TO LOOK BEHIND HIM AND FELT A PAIN IN HIS NECK.
WHILE INSPECTING THE PRODUCTION SHAFT, A TRIP OCCURRED RELATED TO SYNCHRONYZATION/COMMUNICATION OF THE DRIVE WITH THE CONTROLELR. IT TOOK TWO HOURS TO TROUBLE SHOOT AND CLEAR THE FAULT. INVES TIGATION SHOWED NOTHING & A HARD RESET OF THE SAFTY MONITOR & CONTROLLER CLEARED THE FAULT.
A POWER OUTAGE ON EH CIT GRID CAUSED A SHUTDOWN OF ALL OPERATIONS. THE RESTART OF THE PRODUCTION HOIST WAS DELAYED BY THE INEXPERIENCE OF THE ONSHIFT ELECTRICIAN.
EMPLOYEE WAS STANDING ON FLAT BED OF TRUCK REMOVING J HOOKS FROM ROOF TRUCK MOVED FORWARD & EMPLOYEE LOST HIS BALANCE
THE SERVICE CAGE OVERTRAVLED AT THE BOTTOM LANDING. THE TRACK LIMIT SWITHC STOPPED IT APPROX 2' BELOW THE LANDING. THERE WAS A FUEL TRAILER ON THE CAGE. THE HOIST OPERATOR FEARED STALLING T HE HOIST AND HAVING THE CAGE SLIP INTO THE WEDGES. A LUB TRUCK WAS USED TO TRANSFER FUEL OFF THE TRAILER IT TOOK 1 HOUR.
EMPLOYEE WAS BAGGING SALT WHEN HE BUMPED SOME FRAMEWORK OVERHEAD SALT DUST FELL & WENT INTO HIS EYE PRESCRIPTION WAS WRITTEN BY DOCTOR
EMPLOYEE WAS DISMOUNTING HIS SCALING MACHINE WHEN HE FELT PAIN IN HIS GROIN.
PIECE OF DEMOCITION CONCRETE FELL ON EE S FINGER.
EE WAS USING A WRENCH TO TIGHTEN BOLTS ON A TORQUE CONVERTER WHEN THE WRENCH SLIPPED. EE STRUCK HAND ON ANGLE IRON.
