While performing overhead torch work to cut bolts, the miner was struck by hot slag. The slag traveled between the face shield and safety glasses and contacted the eyelid. The miner was evaluated by an ophthalmologist, where a small foreign body was removed and the burn was treated with prescribed ointment.

While manually handling a wear plate during maintenance activities, coworker experienced pain in the left groin area. EE reported the incident and was evaluated by a medical provider. Medical evaluation indicated a possible hernia and follow-up with a specialist was scheduled.

Coworker was walking up stairs to the parts room above the office when EE felt a pop in EE's knee. No slip, trip, or fall occurred. Coworker was evaluated at Nason Occupational Health and was prescribed a hinged knee brace.

Two employees were clearing material from the feeder box. While using hand tools to remove remaining buildup, an employee's right little finger was caught in a pinch point on the feed box cover, resulting in a fracture.

Employee was removing accumulated metal from the magnet. During the removal of a large piece of metal, the angle iron shifted and struck the employee on the left side of the head below the hard hat brim. Employee sustained a laceration requiring a staple and a tetanus shot. The next morning, the Coworker reported dizziness and nausea and was diagnosed with a concussion.

IW was in the process of loosening a hydraulic coupler on a boom cylinder when EE was sprayed with hydraulic fluid. This resulted in a laceration on EE's nose, and hydraulic fluid in EE's eyes.

Laceration to thumb requiring stiches. During LOTOTO, employee stepped into help with project installing v-belts on CV7 conveyor. EE pulled the belt, and it got caught on EE's thumb.

Coworker was removing a screen from the storage rack. The cut edge of the screen slipped from EE's hands and fell forward, striking EE in the forearm. This caused a 3" cut to the coworker and required 4 stitches.

Coworker was changing a troughing idler on conveyor. As they were pulling it out, the idler dropped and their finger was pinched against the frame. Resulting in 5 stitches and restricted duty.

The coworker was carrying 2 flat plates of steel weighing approx. 22lbs combined weight when they felt a pain in right shoulder. They stated that it felt like they were stung with a rubber band. Their shoulder began to bother them and their fingers were getting numb. Health line was contacted and was advised to seek medical attention.

Employee lost balance and fall off of the track to the ground. Employee first made contact with the ground on left shoulder/elbow area. They received 3 stitches in left elbow, has a sprained left shoulder, and a fractured left scapula.

EE was removing a head pulley when it slipped and rolled onto EE's left pinky finger, causing it to get pinched, which resulted in EE receiving two sutures. EE was wearing thick gloves at the time of the incident.

EE was operating D8 bulldozer when EE ran over a large rock. The machine was jarred causing dust to fall off of the roof of the cab. A piece of dust got into EE's eye, causing a scratch to EE's eye.

EE was descending the steps on a haul truck, as EE was climbing down, EE felt a pop in left wrist. EE was diagnosed with a sprain to the left wrist.

Ee's were adjusting the closed side of the crusher opening by pulling a cable connected to a haul truck. As the haul truck pulled on the cable, the cable caught a nearby oil tank causing the cable to lift off the crusher. The flying cable then struck a platform handrail which broke the weld. The broken handrail then hit the injured coworker in the Right Knee.

EE was moving material from a bench to be loaded onto trucks. EE had a full bucket and swung to the left rapidly, causing the rear to lift. EE tried to correct the instability by swinging back to the right, but the machine was too far off balance causing it to overturn on the cab side. In the process of the rescue the ee pulled a muscle in EE's back.

EE was using a 3 rung step ladder to access the engine area on a dump truck. While mounting and dismounting the ladder the ee felt discomfort in groin area. EE went to a health clinic and was diagnosed with a hernia.

A coworker was picking landscaping rocks from a muck pile on a walking surface that was uneven, steep, and unstable. As the coworker stepped forward, the rock under the coworker's right foot rolled and right foot slipped off the rock wedging against a larger rock.

While using a compressed air line with nozzle attached, it slipped from EE's gloved hands striking EE on the lens of the safety glasses and EE's right cheek. The impact caused bruising of EE's left eye.

EE was doing EE's daily routine of pre-trip on a loader. When opening the hood of the loader to check the oil, the hood dropped slightly, and EE experienced pain in EE's right arm/shoulder. EE was diagnosed with a strain of EE's right shoulder and upper arm and was given work restrictions.

The roof of the MCC Room was leaking. Rainwater made contact with the main breaker causing the breaker to trip shutting down the plant. EE strained back while attempting to tarp the roof. First aid measures did not provide relief. Later in the week, ee received medical treatment from a workers compensation panel provider.

EE was in close proximity of another welder, and when EE lifted hood up a piece of debris went up under EE's safety glasses. EE went to a medical facility and a piece of metal was removed from the right eye. EE was not given any restrictions.

A coworker was fabricating a feed box for a wash screw. The work was being done on EE's welding bench. An 18" by 20" plate EE had set on the top shifted and fell about 16", crushing EE's right thumb. The injury resulted in medical treatment.

EE was trying to remove a tree that was stuck at the underside of a crusher, on top of a belt. While pulling on the tree, EE felt pain on the right side of abdomen, and the injury resulted in a strain. The injury did not meet reportable criteria until after EE's follow up visit on 01/22/2024 when ee was issued a work restriction of no shoveling.

EE was operating a 988K PL0105 wheel loader, digging in the muck shot and sending loads to Primary Crusher. As EE was digging in the muck pile, EE hit a tight spot which caused the loader to jar/jolt, and the EE strained EE's lower back.

EE was replacing a relay for the fuel pump truck. EE tried to drill out the new electrical terminal to fit on the existing stud using the drill press. The bit grabbed and spun the terminal, pulling EE's hand into the bit, causing a fracture and a laceration on the tip of EE's left index finger. EE received 12 sutures.

EE was running a loader, loading a truck, when there was a sudden issue with the steering system, and it began jerking to the left and the right. EE was diagnosed with a neck strain. The loader was taken out of service until it gets fixed.

EE was installing a right-side, A motor V-Belt guard on a HP400 Crusher. EE hung a 44" X 37", 20lb piece of guard onto the fastener bolts. As EE was lining up the bottom right-side fastener, the guard came unhooked from top left fastener. This caused the guard to swing down and pinch EE's left hand between the guard and angle support, bruising EE's left hand.

UG mechanic was installing a cylinder on the Gradall Scaler. With the anchor end pinned, the mechanic guided the cylinder to connect the rod end into position. As the mechanic guided the cylinder, EE's left pinky finger was pinned between the cylinder and the frame of the scaler.

EE was using a utility knife, removing strips of belting that was wrapped around a tail pulley. They put a new blade in the knife, and they didn't think it would go through as easy as it did. When the knife passed through the belting, it punctured their left forearm, resulting in them receiving 7 sutures.

EE was working on connecting two pieces of 12"x8' pipeline. EE attempted to move a retaining collar to one end of the piece for connection, the pipe was elevated. EE was in a kneeling position while attempting to move the ring. As EE pulled on the ring, the ring became wedged on the rubber stopping it. The force of the stop caused EE to sprain lower back.

EE had completed housekeeping under the 600 Crusher Feed Tunnel Belt and was exiting from the back side of the belt. As EE planted left foot, it slipped on the wet surface and EE fell to the ground, injuring lower back. The injury resulted in restricted duty.
