Struck by falling object

Coworker was removing a screen from the storage rack. The cut edge of the screen slipped from EE's hands and fell forward, striking EE in the forearm. This caused a 3"" cut to the coworker and required 4 stitches.

Caught in, under or between a moving and a stationary object

Employees were installing a tail pulley on conveyor 13. The tail pulley became wedged between the framework of the conveyor. When an EE pushed on the conveyor to dislodge it, it was loosened and pinched the EEs finger between the pulley and framework. This resulted in a fracture to the EEs right middle finger.

Fall from machine

Employee was standing on a 988 front end loader between the bucket and loader arm standing on the links. Employee stepped back to observe employee's work and fell off the machine, injuring midback/ribs.

EE was stepping down from loader & missed. Step & fell on right shoulder - 3.0' fall.

Emp. using impact wrench to draw up bolt on track. Bolt snapped & tool fell on right hand - caught between tool & machine.

EE WAS REPLACING A PART ON A ""WINDOW REGULATOR."" HE REMOVED THE HANDLE GEAR RELEASING TENSION ON THE SPRING. PLATE ATTACHED TO SPRING SWUNG AROUND & STRIKING PALM OF RT. HAND.
