Worker was re-entering a stockpile truck. He wrapped his right hand around the door frame to close the door. He slammed his hand in the door.
There had been an ice storm that day. Worker was returning to work after a meal break. He was mounting to the cab of a wheel loader. He slipped on one step and struck his upper jaw on a step.
Employee was dismounting from his truck at the scale. He lost his balance and began to fall; he grasped the fisheye mirror to arrest his fall. The mirror broke, lacerating his right hand.
EE WAS DISMOUNTING A BULLDOZER. HIS RIGHT ANKLE HAS AN OLD INJURY THAT DECREASES HIS STABILITY. HE FELL WHEN HIS ANKLE TWISTED. HE STRUCK THE DOZER TRACK FRACTURING HIS RIGHT FEMUR AT THE HIP.

EE WAS ENTERING THE LOADER. A WIND GUST CAUGHT THE DOOR AND CASED HIM TO STEP BACKWARDS ONTO THE GROUND. HE STEPPED ONTO A FROZEN HUMMOCK AND TWISTED HIS FOOT, STRAINING THE LIGAMENTS. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 1/24/00. DID NOT REORT ACCIDENT UNTIL 1/28/00.
