Employee was preforming maintenance tasks on the cone while plant was shut down when ee lost footing on the cone discharge. EE fell from approximately 4 feet high onto shoulder. EE has had shoulder problems in the past and it does become easily dislocated. EE shoulder was in fact dislocated and ee was taken to Medical Center for treatment.

Employee was helping to fabricate a conveyor in the shop. A clamp, which was holding up a piece of metal failed. The metal fell on the employee's right foot, causing a fracture of the right great toe. There was no time lost.
