Employee was standing on ladder when the wind blew the ladder causing them to lose their balance falling and landing on a hand railing.

Employee had EE's arm amputated between EE's elbow and wrist.

Employee was checking on a squeaky roller while roller was in operation. Left hand got caught in roller and tore the skin on the left wrist in two places.

EE was removing the impellor off of a water pump. The impellor fell off of the shaft and onto EE's right arm. It cut EE's forearm open leaving an approximately 3" long wound.

Employee cut the right index finger with a grinder while attempting to clean up workspace. Employee failed to unplug the grinder and hit the trigger when they picked it up.

EE was shoveling underneath a conveyor and EE got a sore back. EE rested over the weekend and Axiom Medical suggested a clinic visit on Monday. EE was then prescribed muscle relaxers.

Employee stepped out of excavator and onto large rock without seeing it causing ankle to roll.

Employee was preforming maintenance tasks on the cone while plant was shut down when ee lost footing on the cone discharge. EE fell from approximately 4 feet high onto shoulder. EE has had shoulder problems in the past and it does become easily dislocated. EE shoulder was in fact dislocated and ee was taken to Medical Center for treatment.

Employee was conducting a pre-inspection of a dozer and was climbing off the machine when employee slipped and fell on the left hip.

Employee slipped on a hill and strained left calf muscle.

Employee was cutting bolts from underneath a screen, raised up knocking hard hat off and hit head on the corner of a piece of metal.

Prying with a Pry bar. Bar slipped and smashed finger between hopper and conveyor.

Opening Hood on a service truck and right knee hyper extended.

Employee was cutting a trelix pad with a utility knife, knife slipped and sliced through glove on left hand, cutting a slice in the palm of EE's hand.

Tripped and fell backwards, hitting lower back on an I-beam.

Employee was moving and throwing rocks by hand and twisted wrong, straining back.

Employees were pulling water line on site in hot weather conditions. Employee went into the gen set, sat down and felt dizzy, light headed and complained of both arms hurting.

Trimming a belt from a stacker. Knife came through the belt when finishing the cut and cut right hand through the glove employee was wearing.

Employee was shoveling dirt to smooth a walkway and evidently fainted. The supervisor found him lying on the ground, called his name, the employee woke up but had no recollection of what happened.

Attempting to straighten a roll of belting and apparently strained back. Employee did not notify the employer until 06/12/2015 stating that it did not hurt until several days later. and therefore did not seek medical attention until the evening of 06/12/2015.

One employee handing a loader tooth to the other employee and the tooth was dropped on the second employee's left foot.

Employee was assisting with changing screens and was tightening a bolt with a wrench and felt a pain in his right arm.

Drove over a couple of ruts in the yard causing the loader to bounce fairly hard. Ruts have since been filled in.

Preparing plant for a move, while taking the stairs off of the generator set, he lost his balance and stairs flipped over on him.

Mechanic from shop was dropping the belly pan of a D8 crawler. The pan slipped and pinched his right thumb between the pan and the ground.

Employee was helping to fabricate a conveyor in the shop. A clamp, which was holding up a piece of metal failed. The metal fell on the employee's right foot, causing a fracture of the right great toe. There was no time lost.

Employee reports low back pain from entering/exiting loader. Chiropractic care sought on 08/11/2010. Anticipated return date of 08/16/2010.

Mechanics were hammaring a retaining bin into a bucket. The hammar glanced off of the pin and struck one mechanic in the mouth, loosening teeth.

Alleged groin pain/discomfort from changing out screens on screen deck, possible hernia.

Employee alleges non-specific right arm pain with no cause given.

Employee stepped on a rock while climbing down from a loader, twisted and rolled his right ankle.

Employee was in the process of cutting a piece of rebar, when the rebar snapped back and hit the employee in the chin and neck, causing deep lacerations.

Injured employee was walking by an area where other employees were doing a belt splice. A metal fragment flew out from the ginding and entered the injured employee's eye.

Changing conveyor belt on a crusher. A piece of belting flew back and hit him above the right eyebrow.

Employee was doing an inspection of a dragline and slipped on some rocks. He fell and sustained a severe strain/sprain to his right foot and ankle.

Individual was delivering items to mine site. He stepped off of a car trailer, his ankle gave way and the ankle rolled. Possible severe strain or sprain.

Getting into a drill truck at mine site, he slipped on entry step, caught himself, but sprained or strained low back.

Changing cutting edges on a CAT 980 G loader and sustained cut to left hand.

Employee was operating a CAT 980 C loader. As the machine dug into the bank of material, the seat failed (later found to be safety wired) and the employee fell backwards.

CLEANING BLOCKAGE OF MATERIAL FROM BOTTOM OF CONVEYOR (VIBRATING FEEDER), HIS AWKWARD BODY POSITION CAUSED STRAIN TO LOWER BACK.

MINER REPORT BACK PAIN ON 9-15-03, BUT NO SPECIFIC WORK RELATED CAUSE GIVEN MINER WENT TO CHIROPRACTOR AND RETURNED TO WORK ON 9-17-03. WORK COMP CLAIM FILED AND MINER REFERED TO MINER COMPLAI NED OF FURTHER BACK PAINS LOST TIME STARTED ON 9/25/03 NOT SURE IF WORK RELATED.

FATAL HEART ATTACK, HISTORY OF HEART PROBLEMS , WAS TAKING NITRO TABLETS.

ATTEMPTING TO DISCONNECT/ UNCOUPLE RAIL CARS HELD COUPLER DISCONNECT TOO LONG AND WEDGED LEFT ARM BETWEEN PICKUP DOOR AND PICKUP.

WORKING ON DRAGLINE/CRANE CABLE, CABLE TWISTED AND SCRATCHED EYE.

DURING PLANT SETUP EE WALKED TO BACK OF CRAWLER TO DO SOMETHING, STOOD UP TOO QUICKLY & HIT HIS HEAD ON RIPPER.

EE WAS USING A CHALLENGER 65 TRACTOR AND DISC FOR SITE PREP WORK. GOV OUT OF TRACTOR TO CLEAR IN FRONT OF DISC AND SLIPPED ON A ROCK.

WAS CHECKING CRUSHER PRIOR TO OPERATION. HAD A DIZZY SPELL, GRABBED FOR LADDER, MISSED & FELL 20-12'.
