While repairing a fender in the shop, EE missed the fender while breaking it loose with the sledgehammer which caused EE's hand to strike the fender. This caused an injury to EE's left pinky nail.

While replacing flashing EE was not wearing gloves and was using a box cutter and was cutting towards self. A witness stated to the employee: shouldn't be cutting towards self.

EE states EE was assisting to clear a flocculent line when EE noticed that the hose line was starting to move as if it was about to fail. EE states EE abruptly turned to run and shut off the air feeding the line, when EE heard the line pop EE turned back to see the extent of the failure. EE believes when EE abruptly turned one way then back the other EE damaged EE's knee.

EE was welding in the HSI crusher. After welding for approximately 4 hours, EE states that a rock "the size of a softball" became dislodged from the crusher above EE and struck EE on head.

While building a temporary staircase at the Lisbon Scale House, EE stepped off the stairs onto unstable gravel. EE caught the edge of the stairwell with EE's left hand for balance, but then slipped another 2', causing EE to strain EE's left shoulder. EE immediately had pain when attempting to rotate EE's shoulder in any direction.

While working under the bucket of a wheel loader noticed that one of the braces against the cylinder of the bucket was unstable. While resolving this, one of the braces came loose and struck EE in the left eyebrow and shoulder, causing EE to fall back into a bolt rack. This caused bruising on EE's shoulder and lower right back with some bleeding from the left eyebrow.

Employee was walking down a hill and turned abruptly to talk with someone behind them and states that the pivoting motion caused a "pop" in their left knee. Employee describes lost range of motion but no pain. Employee had the knee wrapped and went on light duty for the rest of the day, at which time they would go to a local clinic.

A site foreman was struck in the face by a chisel while trying to separate a crusher assembly from its associated hopper feeder.

Miner was maneuvering wooden blocks away from a dozer receiving service, using a hand truck. EE's angle for moving the wooden blocks required EE to duck under some equipment and as EE did so, EE felt pain in EE's lower back.

Two Miners and one supervisor were installing 24" x 25' plastic pipes. Normally equipment is used to place pipes, but this was impossible due to the location of this specific section of pipe. Miners were physically moving the pipe into place by hand. Miner felt lower-right back and leg pain while lifting and pushing pipe into place.

Employee states that ee was driving a quarry truck along a haul route through the quarry when ee hit a pot-hole and the truck bounced. EE felt a pinch in lower right side of back immediately, but that the pain slowly grew over the course of the next week until ee reported the injury on 09/28/2018.

Employee had spend Monday 12/12/16 working on a crushing plant, climbing up and down ladders to track conveyor belts. On Tuesday 12/13/16 EE was climbing up the quarry truck to start it. EE swung EE's leg up on the step and felt a pop in EE's back.

Went up to oil chains on jaw belt and saw that belt that feeds tyrock screen had a lot of build up. Locked out equipment. Got a pick ax and scraper. Walked down the 1-1/4" belt. Scraping the sides of the bin. Used pick ax in corners. Crouched down low and swinging pick ax and got it hooked on an angle. Arm stopped mid swing/felt like a stabbing in front shoulder

Installing oil pressure line on cone. Tightening hose and felt back twinge. Crawled out and told crew he hurt his back. Stayed at work until 9 am and then went home to rest because he could not move. He went to the clinic on 12/9/14. Work status off until Friday when he should be seen for a follow up appointment. Injury: back sprain/strain.

During the previous 3 days (June 16-18), EE had been working on a project pulling a lot of hose. He felt discomfort and pain from the pulling in his groin and behind the belt area. He though it might just have been indigestion, but the pain increased and was reported on 6/19/14.

Miner was stung by an insect on the back and experience an allergic reaction. Miner sought medical attention and received an EpiPen shot. steroid shot, and Benadryl. Miner went home for the day.

Lifted up hatch door over the hydraulic pumps to check hose. Went over and released the pressure off hydraulic tank. Door was straight up & down jammed into postion. Went to check hoses again and leaned on the door. The door cam loose and fell open. Hand slipped and got caught on inside latch of the door.

EE said he was down by the loader, his boss said some things that disturbed him, said he threatened to relieve him of his duties. EE said he got upset, his heart started racing, and he has problems with high blood pressure. He was waiting by the driveway to get a ride and doesn't remember what happened.

Employee was walking down a hill, slipped, and got back up and walked away. He said he was not going to report it, but ended up turning in paperwork on 8/2. He stated he didn't want to report it, but on 8/9 asked about this injury and workers comp so we are now reporting it. He said this event re-injured his back.

On 3/18/10, EE was working on the plant with a skid steer. He had parked and climbed out using 3 points of contact. He put his foot on the tire (that was wet from a puddle) and slipped. He caught himself with his left arm and twisted his right knee. Reported event on 3/19/10 @ 2 pm. Did not seek medical attention until 4/1/10.

EE was standing on the deck of a loader and was cleaning off the snow from the hood. His right foot slipped out from underneath him and he left shoulder fell against the hood of the loader.

EE was splicing a cord (straight line down & away from himself with a circle cut around cord at the end) and he grabbed the cord to pull it off. He was pulling the cord off & towards himself the knife in hand. His hand slipped off cord, & the knife cut his thumb & 1/3 of his tendon. EE got a few stitches on the tendon about 9 on the outside.

EMPLOYEE WAS CHANGING OIL ON CAT MOTOR. HE HAD TO GET MORE OIL & AS HE TURNED AROUND HIS RIGHT FOOT HIT THE LITTLE SLOPE WHERE THERE IS A 2" PIPE ATTATCHED TO WHICH IS A FLAT PIECE OF STEEL HE SLIPPED & HIS LOWER LEG HIT THIS PIECE OF STEEL AS HE FELL. HE HAS A FRACTURED FIBULA.
