The miner was using the hose from a water truck to spray the plant down. The left front suspension air bag of the water truck blew, causing an over exposure of noise to EE's ears. Causing ringing, headache, shooting pain in both ears.

EE was getting out of EE's 982 to use the restroom and stretch. While EE was stretching EE got light headed and fell approximately 6-7 feet off the loader cat walk to the ground below.
The employee slipped on ice and has a sore knee, possible meniscus injury.
The employee was changing the screen on the feeder. The metal plate slipped off the chain and fell on EE's left foot. EE's left foot is bruised and on light duty for a week.

While replacing flashing EE was not wearing gloves and was using a box cutter and was cutting towards self. A witness stated to the employee: shouldn't be cutting towards self.

Employee had spend Monday 12/12/16 working on a crushing plant, climbing up and down ladders to track conveyor belts. On Tuesday 12/13/16 EE was climbing up the quarry truck to start it. EE swung EE's leg up on the step and felt a pop in EE's back.
Employee was welding on a loader bucket with a partner and experienced arc flash. Was using a Miller Welder.

Went up to oil chains on jaw belt and saw that belt that feeds tyrock screen had a lot of build up. Locked out equipment. Got a pick ax and scraper. Walked down the 1-1/4"" belt. Scraping the sides of the bin. Used pick ax in corners. Crouched down low and swinging pick ax and got it hooked on an angle. Arm stopped mid swing/felt like a stabbing in front shoulder

Installing oil pressure line on cone. Tightening hose and felt back twinge. Crawled out and told crew he hurt his back. Stayed at work until 9 am and then went home to rest because he could not move. He went to the clinic on 12/9/14. Work status off until Friday when he should be seen for a follow up appointment. Injury: back sprain/strain.

Lifted up hatch door over the hydraulic pumps to check hose. Went over and released the pressure off hydraulic tank. Door was straight up & down jammed into postion. Went to check hoses again and leaned on the door. The door cam loose and fell open. Hand slipped and got caught on inside latch of the door.

EE said he was down by the loader, his boss said some things that disturbed him, said he threatened to relieve him of his duties. EE said he got upset, his heart started racing, and he has problems with high blood pressure. He was waiting by the driveway to get a ride and doesn't remember what happened.
Employee was walking down a hill, slipped, and got back up and walked away. He said he was not going to report it, but ended up turning in paperwork on 8/2. He stated he didn't want to report it, but on 8/9 asked about this injury and workers comp so we are now reporting it. He said this event re-injured his back.
On 3/18/10, EE was working on the plant with a skid steer. He had parked and climbed out using 3 points of contact. He put his foot on the tire (that was wet from a puddle) and slipped. He caught himself with his left arm and twisted his right knee. Reported event on 3/19/10 @ 2 pm. Did not seek medical attention until 4/1/10.
