Employee was changing a grease zerk on a bucket elevator tail shaft bedring while elevator was in service. Worker's glove was caught in motion sensor star & glove was ripped from right hand causing possible fracture on bone in right hand.

Showing all 10Maintenance men were welding a patch on graltionator elevator and sparks got inside elevator igniting rubber belt & buckets. Fire was extinguished within 30 minutes. There were no injuries. Still assessing damage as of 2/16/12.

GETTING A 4" PIPE OUT AND IT FELL. EMPLOYEE TRIED TO GET AWAY, HE TRIPPED AND THE PIPE LANDED ON TOP OF THE BACK OF HIS LEFT LEG.
