Drowning

The individual was discovered in a structure designed to settle aggregate fines. His job duties involved keeping the area clean from spillage & debris. The Structure wall is over 4 feet high and there was no reason for the individual to have access to the top of the structure. He was discovered floating in the structure.

Employee was using a pry bar to pry out some rocks that got jammed up on the jaw crusher hopper, and when doing so a rock fell onto EE's hand and lacerated EE's left hand ring finger.

lacerated left index finger while working on an air cleaner, lost grip of the air cleaner housing and it fell on their hand.

Employee was removing the bolt on a loader when the bolt made contact with the employees finger causing a laceration that required 9 stitches to close.

Employee was climbing up and down the ladder at the crusher trying to get it started and on the third try employee's foot slipped from the ladder and employee twisted employee's lower back causing a strain.

EE was climbing down a ladder from the control house of the crusher and while doing so EE did not place the arch of foot on the ladder rung but only placed the front of boot where the toe's are and foot slipped and EE caught self with left hand and with the weight of body swung to the right and hit right forearm and elbow on the frame.

Employee was working on repairing a conveyer belt underneath a hopper at face level and while cutting the belt towards himself the cutting knife came toward his face and cut his lower lip and chin. Laceration needed stitches. Coworker was working with him on the other side of the hopper.

The employee was using a utility knife to cut a rubber conveyor belt, while doing so the knife case broke and the blade got out of its case cutting the external side of the right hand (1"" cut). EE got stitches at the hospital and returned to modified duty the same day.

EE was changing shims on a Jaw crusher, when using a hammer to loose a bolt he placed his left foot on a hole at the inner wall(between the jaw and the iron plates used to prevent the jaw to close), while doing so, the safety pin holding the jaw got loose and tramped his left steel toe; the steel toe got deformed and injured his thumb. No fractures where detected at the clinic.

EE was cutting a metal piece at ground level, when getting up changing position he didn't turn the torch off and lost grip of it; he reached it by the hoses and the burning tip got pointed at his right knee causing a 1.5"" burn. EE received first aids at site and refused medical attention. On Jan 27th he asked for Medical treatment, went to the doctor and received modified duty.

As the employee was trying to loosen rocks stuck in the crusher with a metal bar, the bar slipped causing a laceration and fracture to his right pinky when it was caught between the bar and the crusher.
