Still early in shift, workers were using skid steer to clean underneath conveyor. Loader moved by and obstructed lighting, causing skid steer operator to not be able to completely see around him. When loader operator backed out and swung away from conveyor, cleaning plate on bucket of skid steer struck ground man on side of foot.

Worker was assisting maintenance crew to work on a cone crusher. Cone crusher was plugged, so workers tried to release bottom door on crusher. When pressure of rock suddenly forced door down, worker's arm became trapped underneath part of door. Worker suffered bruised arm along with broken bone in arm.

Injured person was in dump truck being loaded. Loader operator loaded truck too quickly and jolt to truck snapped driver's head to the side, injuring his neck.
Employee was walking underneath a stacker when a piece of sand falling from the machine became lodged in his ear. Material from wash plant is often wet and falls from machinery, employee was underneath machine and had wet material fall into his ear. Employee did not seek treatment immediately, but went to doctor two days later to find infection in ear canal.

Employee was kneeling to lift a wooden block (dimensions=12"x12"x2'). The lifting & twisting caused severe back strain, leading him to be taken by ambulance to the nearest hospital. Unknown if proper lifting technique was used or if other methods were available to lift block. Worker was diagnosed with severe back strain and given medication for his injuries.

Moving #6074, and was chaining ladder.EE was backing in the fifthwheel dolly. He stopped to wait for EE and locked parking brake. The loader and the dolly rolled a little bit ahead and ran over back of foot.

EE was conducting pre-shift inspection on Front End Loader & slipped while climbing down. He caught himself with left hand before he fell. When he caught himself he pulled his shoulder. On the day of injury the EE did not think he was hurt.As time went on his shoulder started hurting quite a bit. The EE reported this injury 1/22/07 & sought medical treatment on the same day.

Employee was trying to repair a torn conveyor belt and felt pain in the lower left side of his abdomen. At the time of the incident EE was not certain what was actually wrong and continued working. On 12/6, the office was informed that a specialist had determined EE's injury to be a hernia. On 12/8, EE will undergo surgery to correct this injury.
