Showing all 21Employee was changing belts on motor, when tip of right index finger was caught between belt and pulley fracturing the tip of the right index finger. Local MSHA was office was called within 5 minutes for notification
Employee was working on crusher installing new plate, struck his right hand with a hammer. Did not think he was hurt, did not report injury. 5/2/2014 came to office explained what happened stated was still hurting and was having problems grasping. Called local MSHA office, employee was sent to company doctor to be examined
While stepping over a rock, toe caught. Lost balance and fell, striking knee on ground.
Backed up to stockpile, raised bed. When pulled forward felt frame raised up. Was thrown against passenger door. Thus spraining ankle.
Lifted space heater from back of pickup truck. Carried it approx 15 feet. Weight of heater is approx 45 pounds. Felt sharp pain under left knee cap. Side pressure on knee causes a sharp pain
Brought breaker down and sat it on ladder. Picked up with aid of other employee. Felt stinging sensation in right groin area.
While lifting metal piece from jaw crusher, piece broke loose from chain and was flung into employee's left leg
Employee was decending vehicle using three points of contact. As stepped onto ground left knee felt like something ripped in it. Had previously injured same knee in the past.
GOT SHOVEL OF MUD, BACKED UP, TURNED, THREW MUD INTO HOLE. BACK STARTED TINGLING & GOT TIGHT.
SHOVELING STONE, BENT OVER TO GET ANOTHER SHOVLE FULL. WHEN HE RAISED UP, FELT SOMETHING JERK IN BACK. (SHARP PAIN)
WHILE DIGGING LARGE STONE OUT OF SHOT, STONE FELL ONTO BUCKET OF LOADER THEN SLID OFF CAUSING LOADER TO "BUCK." AFTER ONE OR TWO MINUTES HE FELT A STABBING PAIN BETWEEN HIS SHOULDERS.
AS HE WAS DESCENDING LOADER, FOOT SLIPPED, FELL APPROX 4 FT TRIED TO CATCH HIMSELF WITH LEFT HAND, SPRAINED LEFT HAND.
WHILE CUTTING CONVEYOR BELT WITH UTILITY KNIFE, CUT APPROX ONE INCH LACERATION IN LEFT FOREARM.
EE WAS USING DIGGER BAR TO BUST CLODS IN BIN. CLOD BROKE LOOSE UNEXPECTEDLY. LEFT SHOULDER WAS STRAINED AS A RESULT.
PUTTING BELT COVER ON CONVEYOR BENT OVER & TWISTED BACK.
WHILE STEPPING OUT OF TRAILER LEFT KNEE TWISTED,FELT A POP AND FELL.

TURNING CUTTING EDGE OF KAWASAKI LOADER USING A BAR TO HOLD BOLT IN PLACE WHILE OTHER EMPLOYEE WAS TAKING NUT OFF. BOLT JUMPED OUT, GLANCED OFF BAR AND STRUCK RIGHT HAND MIDDLE DIGIT RESULTING IN LACERATION THAT REQUIRED 5 STITCHES.
WHILE LOADING ELECTRIC HYDRAULIC PUMP OFF OF SERVICE TRUCK BUMPER INTO PLACE IN TRUCK BED, EE FELT PAIN IN BACK WHILE POSITIONING PUMP.
