Employee was changing belts on motor, when tip of right index finger was caught between belt and pulley fracturing the tip of the right index finger. Local MSHA was office was called within 5 minutes for notification

Employee was working on crusher installing new plate, struck his right hand with a hammer. Did not think he was hurt, did not report injury. 5/2/2014 came to office explained what happened stated was still hurting and was having problems grasping. Called local MSHA office, employee was sent to company doctor to be examined

While stepping over a rock, toe caught. Lost balance and fell, striking knee on ground.

Backed up to stockpile, raised bed. When pulled forward felt frame raised up. Was thrown against passenger door. Thus spraining ankle.
