Employee was walking across haul truck from engine compartment to rear frame platform. His feet slipped out from under him causing him to fall landing on metal beam.
Employee was preparing to put oil in his equipment at the start of his shift. He decided to turn on the headlights because it was still dark. After coming down off of the piece of equipment he took a few steps and stepped onto a rock, turning his ankle.
Employee was grinding steel on #801 feeder plate and metal blew in between face and safety glasses. Dust got into right eye.
While opening the lip on the crusher the lid slipped out of his hand and struck his left arm.

THE OPERATOR WAS SORTING MATERIAL. WHILE LIFTING A LARGE ROCK, THE LOADER TILTED FORWARD AND THE ROCK SLIPPED OUT OF THE BUCKET. THE LOADER ROCKED BACKWARD AND THE OPERATOR HIT HIS HEAD ON THE CB RADIO KNOB.
FELL THROUGH A WEAK TRAILER BOARD WHILE SECURING LOAD.
GRINDER KICKED BACK AND STRUCK THE MINER IN THE KNEE.
WHILE LIFTING THE FEEDER MOTOR IN PLACE HE PULLED HIS STOMACH MUSCLES.
WHEN THROWING THE BINDER CHAIN OVER A ROCK ON THE TRAILER, THE CHAIN WAS CAUGHT & HELD THE CHAINFROM GOING OVER THE TRAILER. THE MOTION PULLED HIS LEFT ARM/ELBOW.
EE WAS ROLLING LARGE STONE WITH LOADER WHEN THE STONE ROLLED INTO THE WATER. THE WATER SPASHED ONTO THE WINDSHIELD AND BROKE THE WIND SHIELD. THE GLASS CAME INTO THE CAB AND CUT THE OPERATOR'S FOREHEAD AND SHOULDER.
