Bodily reaction, (Not Elsewhere Classified)

A technician approached a valve set and slightly bent over to check the superheater drain alignment. After checking the alignment, they stood up and walked a few steps. Afterwards, the technician felt a sharp pop and thought their back went out. The Control Room was contacted and respondents assisted injured technician.

Over-exertion (Not Elsewhere Classified)

A Fuel Yard Technician dismounted a wheel loader, when right foot touched the ground, the technician felt a ""pop"" and pain discomfort. The Control Room was notified. Respondents performed an injury assessment, provided first aid treatment and the injured technician was taken to the clinic for additional medical evaluation. The technician was placed on restricted work duty.

A FPD Supervisor's right foot slipped which caused the EE to fall to the floor. Respondents performed a quick initial injury assessment, informed the Control Room & provided first aid treatment. Respondents determined the floor was recently mopped & was still wet. The injured EE was taken to the hospital & was issued prescription medication along with work restrictions.

Struck against stationary object

A ee stepped awkwardly onto a piece of metal grating and experienced a pain in right foot. Technician stated that no medical assistance was necessary. On 3/8/17, the technician experienced additional pain discomfort and the Compliance Manager escorted ee to a medical provider. The technician was provided a prescription with sitting duties only work restriction.

Employee removed glasses to view settings on survey device. Afterwards, EE attempted to don safety glasses, misjudged the clearance and inadvertently inserted the end of the safety glasses into the corner of left eye. A witness informed the injured employee that blood was accumulated in the corner of left eye.

Over-exertion in lifting objects

Lifting beam into place. When the rigging bottomed out the employee lifted the beam to unhook the choker.

Fall onto or against objects

Emp stepped off bay and R foot went down into channel between two bays causing employee to lose balance falling forward. Emp attempted to break fall with hands which slipped off bay causing face to impact horizontal support channel. Emp injuries which included facial fracture of mouth will require 1 tooth to be extracted, 22 stitches to Lip, puncture to R thumb and bruised R knee.

Over-exertion in pulling or pushing objects

Spreader bar was dropped in between tubes to be turned 90 degrees to open gap between tubes for cleaning. Spreader bar was caught between 1st tube in bundle and tube below. Employee tried to remove stuck spreader bar by using force with his arms agitating back and forth without success. Employee retrieved larger spreader bar and separated tube bundle to remove stuck spreader bar.

Employee was moving 4 schedule 40 carbon steel pipe (35lbs) into position at a receiver tank. The employee he felt pain in his stomach but he thought it was something he ate. The employee reported his injury the next day via phone (Sunday 06/23/13) and was taken to the doctor on Monday for examination on Monday 06/24/13 for examination and was diagnosed a Umbilical hernia.

While erecting structural steel components, an employee was using a sleever bar to align holes in preparation for inserting bolts. The employee was pulling the bar with both hands when it became dislodged from the connection point. The sleever bar struck the employee in the face at the nose-cheek juncture. Employee suffered a laceration and broken nose.

Employee was laying steel checkered plate at the hog tower when he felt a pop in his left arm.

Struck by falling object

A millwright was assembling a drag chain by using a crane to stretch out sections of the chain so the next section could be attached to the end. While a section of one of the drag chains was hanging from the crane the rigging came off of the hook and the load fell striking an employee.

Employee was driving rebar stake to establish danger barricade around an excavation. He picked up too high and missed the stake pinching his finger between rebar and driver.

Fall from headframe, derrick or tower

After installing a girder beam in the ESP, all rigging was removed from the crane. The employee was attempting to remove a tag line from the girder beam. He pulled on the rope to see if it was entangled or still tied. The rope broke and the employee fell backwards 10' until his PFAS arrested his fall. During his descent his left foot impacted a walkboard fracturing his left ankle.

Employee was cleaning an area for a weld repair. Employee right eye became irritated with what felt like a foreign object.

EE was welding and stopped to replace the welding rod, picked up his hood and something got in his left eye.

Operating front end loader in coal yard. While scooping a load of coal, tried to roll the bucket back, rear end went up in air. Coal pile gave way, rear end came down hard . The air seat came down and bottomed out. Injured his lower back.

Employee was mowing lawn around office building, grass & dust was blowing around. Later in the day he noticed something in his left eye. He washed out his eye & it felt better, later in the evening he felt something in his eye again. Dust in left eye, irritated , Foreign body left eye, Corneal abrasion. Employee has achieved M.M.I. with no functional limitations.

Oil gun was attempting to retract from the boiler, when it jammed against the igniter. EE lifted up on the igniter to free the oil gun and the gun shot back and pinched his right hand ring finger.

Two employees were moving the Sandpiper Portable Sump Pump out from between the Sump Pumps 1A & 1B. One was pushing the Pump forward while the other was lifting it up with the chain hoist. The Pump started to roll toward Sump Pump 1A; injured tried to hold it back, trapped his (R) hand smashing pinky finger between pumps.

EE WAS CLEANING OUT THE HOPPERS ON THE BAGHOUSE.EE OPENED THE DOOR AND THE MATERIAL WAS PACKED IN TIGHT. WHEN THE MATERIAL WAS BROKE IT STARTED FLOWING OUT, AND THE HOT MATERIAL WENT DOWN INDIDE HIS BOOTS.

EE WAS LEAVING BOILER THRU CHAIN CONVEYOR. HE STEPPED FROM TOP LEVEL ONTO BOTTOM SCAFFOLD BAORD. THE BOARD BROKE AND HE FELL 6 FEET INTO CHAIN CONVEYOR.

EE WAS MOVING A 450# DRUM OF OIL ACROSS SHOP FLOOR WHEN HE CAME TO A NARROW OPEING BETWEEN THE 4160 ROOM, DRUM CART ROLLED OVER A SEAM ON THE FLOOR. DRUM CART HIT THE SEAM IN THE FLOOR & JARRE D THE CATCH LOOSE FROM THE DRUM, ALLOWING DRUM TO MOVE UP & FORWARD.

EE STEPPED ON A PIECE OF AROMA FLEX (BLOCK FROM INSULATION), THAT WAS PARTLY UNDER WATER AND HADMUD ON IT. EE SLIPPED AND FELL ON HIS RT. HIP, WHEN HE FELL HE HIT THE LOCKER ALONG THE POWER HOUSE AND WALL.
