Employee was changing screens and when EE lifted screen off the ground EE felt a pop in EE's left arm. After going to ER it was determined that EE had torn EE's left bicep muscle/ligament.

Showing all 3EE was operating a hydraulic rock breaking hammer attached to an excavator when EE was struck in the face with a flying rock. The machine has same guarding and it is unknown when the screen was removed or why. The EE didn't notify anyone in authority and left jobsite early, and later on is when boss talked to EE on phone that night. Never did tell boss of injury.
A rock was stuck in the frame work of an unused conveyor. Mechanic was trying to pry rock out when bar and rock slipped pinching finger against framework, resulting in two fingertips being smashed.
