Employee was changing screens and when EE lifted screen off the ground EE felt a pop in EE's left arm. After going to ER it was determined that EE had torn EE's left bicep muscle/ligament.

MACHINERY
Kincheloe & Sons Inc · Struck by flying object
EE was operating a hydraulic rock breaking hammer attached to an excavator when EE was struck in the face with a flying rock. The machine has same guarding and it is unknown when the screen was removed or why. The EE didn't notify anyone in authority and left jobsite early, and later on is when boss talked to EE on phone that night. Never did tell boss of injury.
