The employee was cutting steel using a carbon electrode. During the operation, the employee temporarily removed the face shield, allowing a particle generated by the cutting process to enter the left eye. The task was being performed under normal operating conditions.

During corrective maintenance on a conveyor belt system, the employee was aligning the belt and cleaning rollers. The conveyor remained energized and operating, with mechanical guards removed and improper tools in use. The employee's left forearm became caught between the belt and a roller.

The worker was waiting for a container to be filled with degreaser. Although EE initially had EE's safety glasses on, EE removed them upon entering the room because the area was darker. During the filling process, the pump malfunctioned and the hose detached due to inadequate fastening, causing the product to splash directly onto the worker's face and eyes.

During the operation of a track drill, one of the drill bits was positioned outside the storage channel. The operator requested EE's coworker to activate the drill changer lever while EE manually held the bit. Upon activation, the operator's right thumb was caught between two drill bits, resulting in an injury.

The affected worker began workday at 6:30 AM. While loosening a screw on the machine, EE lost control of the tool EE was using, causing EE to hit forehead, specifically on the left side, at the level of the eyebrow. As a result, EE sustained trauma and a wound that required a three-stitch suture.
The accident occurred while the miner was attempting to adjust a piece on a forklift. The piece rolled over, causing minor injury to fingers #2 and #3 on EE's right hand. Finger #3 sustained a small amount of bleeding, with no significant impairment reported.

At the time of the incident, the miner was preparing to replace a wiper on a conveyor. To perform this task, they passed under a different conveyor, unfortunately, they were not wearing their protective helmet. When they lifted their head before passing through, they struck it, causing a laceration that required (7) stitches.

While operating the loader, the miner began experiencing lower back pain when attempting to dismount from the equipment.

Employee was operating the hydraulic hammer to break a rock in the primary crusher and a hydraulic hose failed, being hit with the hydraulic fluid in the chest and splashed in the left eye.
Replacing a blade from a porta blade, the miner missed sliding the blade and cut right thumb. As a result of the incident, the miner received (7) seven stitches.

The truck operator was driving from the material extraction point to the crushing plant when apparently lost the vehicle control and impacted the berm and as consequence overturned.

When the miner removed a piece of metal from the magnet, the magnet immediately pulled back the metal simultaneously and pricked the thumb of the right hand.
The miner did not notice that a pick-up truck was driving in reverse and was hit by the side of the truck. The driver of the truck stopped immediately once he notice the miner was in the ground. The truck was subject to inspection by MSHA personnel September 4,2014 and there was no deficiencies.

An independent truck driver fell to the ground at the office parking lot. He die from natural cause (Heart Attack). The personnel present at the moment of the incident did not believe this dead was reportable. We called the San Juan District office to ensure we were in compliance with the rules and regulations.

He was mounting the jaw die of the jaw crusher, they were set with a wooden piece, opened the jaw crusher to align the screws, apparently one of the wooden pieces fell causing the jaw die to fall forward and wedge his right foot.
