The worker was waiting for a container to be filled with degreaser. Although EE initially had EE's safety glasses on, EE removed them upon entering the room because the area was darker. During the filling process, the pump malfunctioned and the hose detached due to inadequate fastening, causing the product to splash directly onto the worker's face and eyes.

EE fell from the top of the engine, sustaining a nasal fracture and a laceration to EE's forehead.
During the operation of a track drill, one of the drill bits was positioned outside the storage channel. The operator requested EE's coworker to activate the drill changer lever while EE manually held the bit. Upon activation, the operator's right thumb was caught between two drill bits, resulting in an injury.
The worker did not maintain a safe distance from the operating crusher, exposing self to flying rocks, which have contributed to the incident.
The affected worker began workday at 6:30 AM. While loosening a screw on the machine, EE lost control of the tool EE was using, causing EE to hit forehead, specifically on the left side, at the level of the eyebrow. As a result, EE sustained trauma and a wound that required a three-stitch suture.
The accident occurred while the miner was attempting to adjust a piece on a forklift. The piece rolled over, causing minor injury to fingers #2 and #3 on EE's right hand. Finger #3 sustained a small amount of bleeding, with no significant impairment reported.

At the time of the incident, the miner was preparing to replace a wiper on a conveyor. To perform this task, they passed under a different conveyor, unfortunately, they were not wearing their protective helmet. When they lifted their head before passing through, they struck it, causing a laceration that required (7) stitches.
While operating the loader, the miner began experiencing lower back pain when attempting to dismount from the equipment.

Employee was operating the hydraulic hammer to break a rock in the primary crusher and a hydraulic hose failed, being hit with the hydraulic fluid in the chest and splashed in the left eye.
The miner alleges that EE was pumping diesel into equipment and when EE got off a wall EE fell on a stone and sprained EE's ankle.

An independent truck driver fell to the ground at the office parking lot. He die from natural cause (Heart Attack). The personnel present at the moment of the incident did not believe this dead was reportable. We called the San Juan District office to ensure we were in compliance with the rules and regulations.

He was mounting the jaw die of the jaw crusher, they were set with a wooden piece, opened the jaw crusher to align the screws, apparently one of the wooden pieces fell causing the jaw die to fall forward and wedge his right foot.

EE was trying to position the bulldozer blade vertically with a loader. when the loader went back, the blade fell down and he was thrown out of the loader bucket and fell from an altitude of 10 ft.
WHILE EE WAS WORKING ON A CONCRETE FOUNDATION BENEATH THE SIDE OF A CONVEYOR, THERE WAS A POWER FAILURE. WHEN THE POWER CAME BACK, THE CONVEYOR STARTED TO OPERATE, MAKING SOME ROCKS FALL DOWN, HITTING EE ON HIS BACK.
WHILE THE MINER WAS DRIVING A CAT 769C (DUMP TRUCK) LOADED WITH STONES UP A HILL THE TRUCK STARTED TO LOSE POWER AND HE STOPPED, THEN THE THE DRIVER TRIED AGAIN TO CONTINUE, BUT THE ENGINE OF THE TRUCK STALLED. HE TRIED TO BACK IT UP AGAINST THE EMBANKMENT AND WHILE DOING SO, HE TURNED OVER TO THE SIDE.

THE EE WAS USING A HYDRAULIC JACK TO OPEN THE SIDE BOARD OF THE VIBRATING SCREEN, TO INSTALL A DECK FRAME; WHILE UNDER HYDRAULIC PRESSURE THEY ""BANGED"" THE SIDE OF THE FRAME WITH A SLEDGE HAM MER. THE IMPACT CAUSED THE HYDRAULIC JACK TO JUMP HITTING THE PIPE HANDLE, CUTTING THE FINGER OF THE EE.
EMPLOYEE WAS TRYING TO INSTALL A TENSION SPRING ON THE EXCAVATOR EX-1-002, THE SPRING BOUNCED CAUSING INJURIES ON ONE OF HIS FINGERS AND ON A TOOTH.
EE WAS DRIVING CO TRUCK WITHOUT PERMISSION. THIS TRUCK HAD TRANSMISSION PROBLEMS AND WAS STOPPED& OUT OF SERVICE IN OUR FACILITY. SUPERVISORS & MECHANICS ADVISED HIM NOT TO DRIVE IT UNTIL THEY FIXED IT. HE DIDN'T CARE AND USED IT CAUSING A FIRE ON THE VEHICLE. WHEN EE TRIED TO COMBAT THE FIRE, STEAM AFFECTED HIS FACE PRODUCING IRRITATION. THE CO HAS SUSPENDED THE EE FOR 2 WEEKS.
EE WAS CHANGING JAW DIES AND ADJUSTING SCREW THE MULTIPLIER CAUGHT PART OF HIS MIDDLE FINGER ON RIGHT HAND CAUSING BLEEDING.

THE EE WAS WORKING ON THE HYDRAULIC SYSTEM OF A LOADER WHEN HE LOOSENED UP A UNIT, HE LOST HIS BALANCE FALLING ON HIS BACK HAVING A KNEE INJURY.
EE WAS DRILLING A HOLE INTO METAL TIP OF BATTERY CABLE TO MAKE THE HOLE BIGGER. WHEN DOING SO THE GIMLET GOT STUCK AND THE METAL TIP STARTED TO SPIN. EE WAS HOLDING DOWN THE TIP WITH HIS FINGE R AND IT GOT HIT BY THE TIP AS IT SPUN.
HE WAS STANDING ON THE D-9 TRACKS BECAUSE HE WAS INSTALLING A SCREW ON ONE OF THE DOORS. WHEN HE MOVED HIS LEFT FOOT SLIPPED, & HIS LEG WAS NOW CAUGHT BETWEEN THE TRACKS & THE BLADES SUPPORT. ALL THE BODY WEIGHT WAS ON HIS LEFT LEG & THIS WAS ON HIS LEFT LEG & THIS WAS THE CAUSE OF THE INJURY.

WHILE OPERATING THE TRACK DRILL STARTED TO RAIN HEAVILY, SO HE DECIDED TO MOVE FROM THE SITE GOING BACKWARDS. NOT NOTICING THAT THE RIGHT SIDE OF THE TRACK DRILL WAS OFF THE TRAIL CAUSING THE TACK DRILL TO ROLL OVER.
WHILE PAINTING A HOPPER A FALLING ROCK ABOUT 1 1/2"" FROM A CONVEYER HIT HIM AT THE WRIST. THE WORKER HAD A PREVIOUS FRACTURE AT THAT WRIST.
WHILE WORKING WITH A BLASTER, HE FELT A SHARP PAIN IN HIS RIGHT ARM WHICH CAUSED HIM TO STOP WORKING. THE EMPLOYEE COMPLAINED OF PAIN IN HIS RIGHT ARM APPROX ONE MONTH BEFORE THIS ACCIDENT.
THE OPERATOR WAS PREPARING TO MOUNT INTO THE CABOF THE TRACK DRILL AND A STRONG WIND BLEW THE DOOR, PINCHING HIS FINGER AND PRODUCING A CUT.
