Truck driver sitting in cab with window down. Another truck backed in next to the truck driver & dumped large load of rocks. A small piece of rock splintered off and struck seated driver on left side of head causing a laceration that needed a staple to close. Driver was treated and released.

Miner used a shovel to clean mud build up on bottom of return belt (while running) shovel contacted belt pushed back twisting the fingers on left hand causing 3 fractured fingers and two lacerations
loader operator was unlocking gate to perform snow removal and slipped and fell on r ankle breaking tibia and fracturing fibula treated at cape cod hospital taken by ambulance.
Operator was climbing up the loader and strained left knee. There were no changes or alterations to EE's machine.

The mine slope collapsed trapping our miner in his machine. The recovery effort was not successful and the employee expired at the mine site.
Our driver injured his back assisting in the recovery efforts to help another miner.
Installing v-belts on sheave, while turning sheave by hand thumb got caught between v-belt & sheave.
Miner was greasing head pulley on screen plant. He fell off the plant frame approximately 6' to the ground surface, landing on his back, he laid on the ground conscious but in pain. He was transported by ambulance to Jordan Hospital ER. He was released later that day. He had severe bruising that kept him out of work the remainder of the week. He returned to work on 9/30/13
Employee was dragging a screen deck across the ground and strained his right shoulder.
Blocks that support the plant were rotten. Plant was bouncing. Even tho' the shaker was off, the bouncing caused a rock to fall into running crusher. The jaw projected the rock out of the hopper and the rock hit ee 20' from the plant on the ground.

EE WAS WALKING WITH PART & WAS BENT OVER DUE TO LOW CEILING HIS HARD HAT WAS KNOCKED OFF BY CEILING EE BENT DOWN TO RETRIEVE HIS HAT & HIT HIS HEAD ON A LIGHT FIXTURE WHEN HE STOOD UP
