Employee was filling water tanks on back of truck. Water pressure was too high and employee lost control of hose causing them to lose balance and fell off the side of the truck. When employee fell they struck their head on the concrete knocking them unconscious.
Employee was cutting a piece of belting for a skirt rubber with a razor knife and slipped and cut EE's left thumb requiring two stitches.
Employee was rigging a lifting chain to a section of conveyor that was connected to the mini excavator and the operator moved the excavator stick and pinched employee lower arm between conveyor section and excavator stick and bruised lower arm.
Employee was standing in office during a pre shift meeting and passed out briefly and fell to the floor hitting their head on printer on the desk, employee started responding to Foreman fifteen to twenty seconds later. EMT'S were called and employee was taken to be checked out and released two hours later with Return to work paperwork and diagnosed with Dehydration.
Operator shut down the FEL for the day. As the operator was stepping down from the last step of the ladder to the ground, EE rolled their right ankle and was advised to wear a walking boot for 2-3 weeks or till it felt better. EE wore it for four days and is wearing regular work boot now.
EMPLOYEE WAS REPLACING THE SEAT IN THE TANDEM ROUGH TRUCK AND TWISTED SHOULDER AND STARTED TO FALL FROM STEP ON THE SIDE OF TRUCK AND GRABBED TO STOP AND POPPED EE'S SHOULDER.
EMPLOYEE WAS HELPING MECHANIC FLIP FORKS OVER ON BOBCAT SKIDSTEER AND THOUGHT EE PULLED A MUSCLE. NOW WENT TO DOCTOR BECAUSE IT WAS NOT GETTING BETTER AND WAS TOLD EE HAS A HERNIA.
Employee was climbing down steps of front end loader and stepped to ground and twisted EE's ankle.
Employee stated that the guard from the dredge winch was removed to replace the cable, at some point the cable started to slide off the deck into the water and EE jumped over the guard to stop it and cut the back of EE's leg on the guard.
Employee was loading a skid steer tire in back of pickup and felt a sharp pain in groin area. Employee did not seek medical attention right away. Employee now had surgery on 10/17/2019 for a hernia.
Employee was operating the dragline and turned to dump bucket and felt machine tilt towards water and panicked and jumped off on to ground and twisted left knee. Dragline did partially go into water on one side.
EMPLOYEE WAS CHECKING A PLUGGED SAND PUMP AND STEPPED ON A HATCH COVER THAT WAS NOT SECURED, FALLING DOWN AND SCRAPED FOREARM, SHIN AND TWISTED LOWER BACK. EMPLOYEE CONTINUED TO WORK UNTIL 06/19/2019 WHEN STARTED HAVING PROBLEMS WITH LEFT SHOULDER AND HAD IT CHECKED AND WAS TOLD IT WAS A SPRAIN AND PUT ON RESTRICTIONS OF RIGHT HAND USE ONLY.
Employee was moving a bearing housing for installation on the screen plant and pinched gloved finger between two one hundred pound housings. When they removed glove they realized that they had severely cut a finger right behind the finger nail. Several days later the finger tip was removed.
Flipping piece of screen rail over to knock pins out. Rail fell causing hand to be pinched took glove off saw bloody finger and call for supervisor.
Was installing a tail pulley hopper and it fell and pinched right hand between hopper and guard. causing fracture and laceration to tip of middle finger.
Employee was removing steps from crusher platform and they fell on employee's foot.
Employee was entering mill elevator and door slammed shut on hand and injured one of the fingers on left hand that resulted in stitches.
Rock became lodged in vibrating feeder, shut down equipment and locked out. Employee went in feeder to dislodge rock. When doing so employee's foot was hit by metal skirt board bouncing off steel toe boot and broke little toe.
Employee was in manlift with another employee welding, lifted his welding shield and other employee was grinding and a small piece of grindings hit his eye. Employee didn't realize anything was in his eye until the next day.
Changing screen cloth, lost balance and fell into hand rail.
Working on skid loader attachment and stepped backward. Tripped over some steel and fell on his rib cage.

Was standing on work boat, lining a pipe up and stepped back off boat and hyper extended left knee.
Employee was walking and slipped on ice a felt a pop on ankle. Fibula fracture.
EE was loading customer trucks from the 411 stockpile and the face of the pile slid down on loader bucket causing the rear of the loader to rise and then EE dropped the bucket, causing the rear of the loader to slam down and bounce several times. EE complained of back pain then.
EMPLOYEE CLAIMS TO HAVE SPRAINED HIS BACK WHILE RUNNING END LOADER IN SHOT ROCK AREA
Writing weights on pallets of flagstone and bent over to pick up a marker and felt a sharp pain in his lower back.
Connecting ladder chain together and cutting head flopped over and pinched EE's finger tip causing a laceration and fractured finger tip above his knuckle.
EE was helping install a punch plate on the screen deck and had his fingers around the plates opening as it was being pushed into place. Caught finger on cross rail, ripping open end of finger, which had to be sewn back together at hospital.
EE was cutting crown rubber on screen plant and cut his finger on his left hand. This resulted in 5 stitches to his finger.
Employee was trying to cut off a frozen water hose to repair a frozen valve and slipped with the razor knife and cut his thumb. Employee received 6 stitches.
EE's reported to work and discovered the electircal trailer and control tower was on fire. Supervisor called fire department and MSHA then.
Employee was loading stone from the mill. When he opened the truck door to open the bin gate the truck rolled back and caught his left arm between door and mill frame. Employee thought truck was in gear but it was not.
Employee was running over to shut off belt that was slipping due to rain. Employee tripped on rock and twisted ankle. X-rays showed a small chip broken in right ankle.
Employee was on the 4th floor of the mill cleaning the 3' X 8'chute with a poking rod. He ran his hand into the end of the chute, penetrating the skin between his 2nd & 3rd knuckles. As it was a little deep he went to the hospital & received 6 stitches & a tetanus shot.
Employee was picking up a piece of flagstone to put on a pallet and felt a pop in his lower back.
While moving oxygen tank it slipped on floor of utility truck and landed on finger, breaking it.
EE was stacking flagstone off the conveyor belt when he picked up a piece of stone it split in half, when EE reched to grab it he caught his finger between the stone and belt head on side of conveyor belt. When he took his glove off he had cuts on the top and bottom of his right index finger.
The employee was stacking flagstone on a pallet when a piece of stone slid off an adjacent pallet and struck him in the back of the right knee causing him to twist and strain his knee. The injury caused swelling and pain.
DRIVING TRUCK TO MOVE BOULDERS ALL DAY WITH WINDOWS DOWN, HE STARTED FEELING SICK IN THE AFTERNOON. THERE WAS A HOLE IN THE FLOOR BOARD OF THE TRUCK. HE WENT TO THE HOSPITAL BUT THEY DID NOT DO A BLOOD TEST TO SEE IF HE DID HAVE CARBON MONOXIDE POISONING. THEY DID A CAT SCAN TO RULE OUT TUMOR.
None, Steps were clean of debri, handrails were used in a 3 point contract left foot caught grib strut, twisting knee.
EE WAS STACKING FLAGSTONE & ANOTHER EE WAS UNPLUGGING HOPPER WITH THE LOADER WITH FORKS & A 6" PIECE OF FLAGSTONE FLEW OUT & HIT INJ EE IN THE RIGHT CENTER OF HIS BACK.
EE WAS WRITING PRODUCT NAME AND WEIGHT ON PALLETS AND A COWORKER IN THE FRONT-END LOADER WAS PICKING UP PALLETS AND MOVING THEM FOR AN ORDER. THE COWORKER CAME IN TO PICK UP A PALLET AND THE FORKS WERE A LITTLE OFF TRACK AND PUSHED THE PALLET AND ROCK INTO EE, HITTING HIM IN THE RIGHT ARM AND BACK.
WAS UNPLUGGING A CHUTE WITH A SPUD BAR AND IT CAME BACK AND PINCHED MY LEFT HAND AGAINST AN I-BEAM.
EE WAS CLIMBING DOWN LADDER OF HAUL TRUCK, HE SLIPPED AND STARTED TO FALL, HE THEN GRABBED THE HAND RAIL, THEN HIT HIS RIBS ON THE STEP, GRABBING HANDRAIL WHEN FALLING WHICH SPRAINED HIS VERTABRATE IN HIS BACK.
EMPLOYEE WAS TIGHTENING DRIVE BELTS ON CONVEYER.RING FINGER ON RIGHT HAND GOT CAUGHT IN END WRENCH BROKE FINGER JUST ABOVE THE KNUCKLE.
GRINDING SLAG & BOUNCED OFF WALL TOWARDS EE.
EE WAS CLIMBING UP THE LADDER TO FIX THE TARP. HE GRABBED THE WRONG SECTION OF THE LADDER. HE GRABBED THE TOP SECTION OF LADDER WITHOUT THE RUBBER FEET. WHEN HE CLIMBED UP THE LADDER IT SLID O UT FROM UNDER HIM & HE FELL (STILL ON THE LADDER) TO THE CONCRETE FLOOR. HE HIT HIS ELBOW, BRUISING IT.
