A conveyor belt had stopped due to build up. It was stopped & locked out. An employee climbed onto the belt to clean it out. When the material had been cleared, the belt gave way and moved toward the tail pull, pulling the employee between the belt and water bar bracket.

Injured Miner was using a pry bar to remove a blockage from the secondary crusher. While using the pry bar, he felt a "pop" in his right shoulder. On 1/24/2009 the Dr. confirmed a dislocation of the right shoulder with a possible rotartor cuff tear.

EE WAS GETTING OUT OF BED OF POWDER TRUCK. HIS FOOT SLIPPED OFF OF STEP AND HE STRUCK HIS FACE ON THE SIDE OF THE TRUCK CUTTING HIS NOSE AND LIP. HE REQUIRED APPROX. 15 STITCHES FOR 60TH WOUND S.

EE ATTEMPTED TO ENTER A MANLIFT WHICH WAS PARKED UNDER A CRUSHER FEED BIN. OTHER WORKERS WERE CUTTING A PIECE OF 1/2" SHEET STEEL TO MODIFY THE CRUSHER FEED BIN. HE WAS NOT WEARING A REQUIRED HARD HAT. WHEN HE WALKED UNDER THE CRUSHER FEED BIN, A PIECE OF STEEL FELL, STRIKING THE LEFT SIDE OF HIS SCALP. HE HAD 5 STITCHES AND WAS RELEASED TO WORK IMMEDIATELY.
