Employee was operating drill and rock broke off and hit right hand.

EE was swinging sledge hammer when EE hit leg instead of wedge in granite.
Crane hook caught employee's shirt causing them to trip and fall. When they fell, leg hit outrigger. EE didn't go to the doctor until Monday, April 2, 2018 due to the fact they were hoping it would get better over time, but it actually became more sore and agitated as EE continued to be active on the leg. Hence the reason for the delay in seeking medical attention.
Employee was pushing rock out when the jack hammer slid off and hit hand mashing fingers.
The quarry crew was putting up the Christmas tree. Employee was tapping on the wedge used to stabilize equipment when a piece of the wedge chipped off and went through pants leg and penetrated leg.

Opening powder magazine and door slipped from hand and mashed fingers on his hand. Cut tip of thumb almost off, doctors reattached.
Employee said he slipped and hurt his back.
EE complained of tightness in chest & pain in arm Ambulance was called . ER said he was dehydrated & gave 2 bags of fluid & released. ER said he needed 3 days off & recommended he contact his personal doctor reguarding stress test.
