I was cleaning the 2nd deck of a 3 deck screen with a bar. The bar got lodged between 2 wires of the screen, I hurt my left shoulder dislodging it.

material was stuck to apron screen deck that drops into the cone. Employee was cleaning the material off with bar, swung bar too far in to get to the back corner and got top of tampering bar too close to overhead conveyor belt. Bar was grabbed by belt and pulled in, thumb was pinched between bar and cross member and resulting in the tip of the thumb being pulled off.
EE was operating the front end loader stopped to get out and stepped down on uneven ground twisting ankle.
EMPLOYEE WAS INSTALLING CHUTE BRAKET ASSEMBLY & SMASHED/JAMMED TWO FINGERS OF RIGHT HAND.
